Cortisol testing

I had my cortisol tested with BH and the results were high, see attached results. Endo wasn't interested although I thought it was another piece in the puzzle of me trying to find out why I don't feel much better on T4/T3 which I have been on since last year this time. Initially I felt better but have again been putting on weight, have palpitations, at night time mostly and generally feel fatigued. On my request she gave me a 24hour urine test but declined to go into what else might be going on or what the consequences of high cortisol could mean for my health. The result have no one back and they state '24hour cortisol was less than 40nmol/l, which would exclude Cushing's syndrome'. No other explanations given or what should happen next. Has anyone had experience with cortisol testing and what it could mean? Any suggestions greatly appreciated, as always.
