Yes, TSH fluctuates thoughout the day and night and seasonally, being lower spring/summer and higher in the autumn and winter. The graph in the link below shows how TSH fluctuates.
Mine fluctuates quite a bit and has done from before I was on meds (when it went from 30ish to 5ish).
When I was first diagnosed my tsh was 56 I commenced levothyroxine 50 mcg, after 3-4 weeks felt much better. My repeat tsh was within normal range! My gp was surprised but said repeat bloods in 1 yr. I went downhill again about 3-4 weeks after this and my repeat tsh was 13 . My gp said " you're right you are hypo again" . I thought that once you're on treatment you would remain stable but obviously not , so I look out for symptoms daily. My tsh is now 4.2. I,m on levothyroxine 75 mcg no further action required .
You should have bloods done every 6-8 weeks (with a dose adjustment if necessary) until you are symptom free, which usually means TSH under 1 and Free T4 and Free T3 in the top quarters of their ranges. TSH 4.2 probably means you are still undermedicated, but the important thing is how you feel. The worst thing a GP can do is to adjust the dose according to the TSH result alone.
