Practitioner number for saliva testing

I want to order another Genova UK salivary adrenal profile. I can't immediately get hold of my ND to get her practitioner number. I thought we could cite ThyroidUK as the practitioner but the online order form requires a 5-digit number. Can anyone tell me what that is please? Thanks.
