when first diagnosed with underactive thyroid I was put on a combination of T3 and T4 without any problems after 10 years it was changed to just T4. for the past 5 years on just T4 my levels have been all at sea and weight gain is a major problem, should I asked my endo to revert me back to the T3 T4 combination, any suggestions please

Definitely ask to be changed back to T4/T3, particularly since you did well before.
The weight gain could also be due to your metabolism being lowered due to the change from T3/T4. As well, he should be aiming for your TSH to be around 1 or below or suppressed (whichever makes you feel well). If our TSH is kept in the 'normal' range we can get other medical problems.

P.S. when you have a blood test for your thyroid gland it should be as early as possible.Don't take levo before it take it afterwards as it can skew the results. There should be roughly a 24 hour gap between levo and blood test.
Levo should be taken first thing on an empty stomach and don't eat for around an hour, so that food doesn't interfere with the uptake of medication. Supplements/medications should be taken 4 hours apart.
Do you have some recent blood test results you can post here? It is possible levo alone will never help you, or you may just not be taking enough, which will be a problem with or without t3. Like you, I didn't feel well on levo alone.
Did they give you any reason why they discontinued t3?
