IBS & Haemorrhoids , Thrush , pain & discomfort - no life

Hi there , I have had many health issues in the past , short term and long term but this is really getting me down . I have been suffering with piles for nearly 3 months now . Firstly it started after taking antibiotics for a suspected bladder/kidney infection, leaving me constipated and resulting in the piles , and thrush. They are mainly very small tags or lumps ( not thrombosed ) externally but they are extremely uncomfortable . I also have vaginal and perineal pain and burning. I have seen a GP who after examination stated no internal piles could be felt but then diagnosed Thrush as the main cause of the discomfort . After treatment , my issues are still the same and have not cleared . I am now getting to rock bottom ( excuse the pun ) , I'm going commando for most of the time , and luckily I dont have to go out to work . I have had IBS for many years but never had issues like this until this year. I'm 66 and have been on HRT since the age of 38 when I had a total hysterectomy due to Endometriosis. The last Thrush treatment has had no effect , and the piles are still appearing . I feel like I've a marble stuck between my butt cheeks even though they are very small , and there is constant pressure when sitting down . I have had no bleeding at all . I could just sit and cry some days , and I dont know what to do for the best . The Gp doesnt seem that interested really , but then its not her having to deal with it on a daily basis . It's costing me a fortune in trying different meds and I dont know whether to treat the haemorrhoids or the diagnosed Thrush or both anymore . I was tested for diabetes , bv and the Trich infection ( sorry cant remember the full name ) and they all came back negative . I am hoping that someone out there can help in suggesting something else that may help . I am using sitz baths , cold gel packs and are waiting for a course of vitaflora tablets to arrive as there is a possibility that if it is mostly down to chronic thrush and uti infections that it could be due to Vaginal atrophy. My hrt has also been increased . Sorry this is a long post and would probably be suitable for various forums in the group but I have to start somewhere . Hopefully someone can help , but at the same time I'm hoping no-one else is going through this .

Am sorry to hear what you going through, I don’t have a solution because I have a very similar situation. GP examined me internal lot and said nothing there but I feel there is, did feel like a golf ball now feel likes marbles. They only small relief I have is pile cream as it numbs it slightly.

If you fine an answer please let me know.

Thanks for your reply , sorry to hear you are going through the same sort of thing . Its difficult for me to decide what to treat atm , front or back . But I am red raw sore now at the front and have lower abdominal pain as well as a pain in the butt. I was given some suppositories for the back passage from the gp but have not used them as I am a little worried about the ingredients as I react badly to a lot of meds . Tonight I can see visible signs of thrush so have decided to blast it with everything I can . It's been a recurrent problem for the past year and my regular gp gave me a strong course to follow that usually works . The last gp I saw recently wasnt my normal gp ( a locum )and I was surprised she actually offered me an in person appointment as I wouldnt have normally got one as we are on telephone consultations due to the pandemic. She gave me a different course which at first was appearing to work but then obviously didnt clear it enough and its come back with a vengeance.

My piles were pretty much the same feeling as yours , felt like a large swelling at first rather than external lumps , once the swelling started to subside the smaller lumps and tags started to appear , Although not looking like grapes ( thrombosed ) just pink , they were very painful to touch . I have been using creams internally and externally and regular sitz baths . This has given me some relief but still not cleared and they are reducing but then another appears , its like they are hidden by the swelling.

I have read that piles just inside the opening cant be felt with the digital examination , not sure if this is true but I cant see how you wouldn't feel them if there were any as they can obviously be felt when they are on the outside .

I am hoping that my IBS settles down quickly as this definately doesnt help .

This is what I am using /used - Anusol Cream , Natural sprays and creams for numbing , Anacare , Sitz baths of warm water ( Ive heard that you can add apple cider vinegar or baking soda for a better / longer effect but not tried this ). and Ive also purchased some small gel packs that are segment shaped that you can freeze and put between your butt cheeks to help with the pain and reduce swelling . I found these do help to some extent but you cant use them for long.

It is so debilitating , and stressful at the moment . I have often heard people laughing and making a joke of piles but this definately is no joke .

I hope you can find some relief in the near future .

Hi Bassem1979 I had a really bad night last night , woke at 3am in lots of pain and burning from the hemorrhoids so I decided to try out the suppositories that the gp had given me . I wasnt sure about using them like I said previously as I thought I may react badly to them but the pain was so bad I didnt know what else to do . It inserted ok without any issues , didnt seem to hit anything on the way up so I think the lumps are mainly external or just within the entrance. Anyway , after 30 mins or so it started getting easier and I managed to fall back asleep . Ive used another this morning and this is the best I've felt for a long while . Yes they still hurt but I can sit a lot more comfortably . Not sure if you've already tried them but they are called Scheriproct Suppositories and contain Prednisolone hexanoate/ Cinchocaine Hydrochloride. I was given 12 in a box under the instructions of use when needed, but the instructions say use one a day or after bowel movement , or if pain is severe you can use 2 for the first couple of days but dont use longer than 7 days . You can also buy similar ones online. Thought the info might be of use to you .

Hello JayBee

I have gone through some very similar things and I am about the same age, 65. Though I have been dealing with IBS for over thirty years it got out of control after being put on antibiotics for a different issue for over six months. After that i had many of the same problems as you, thrush and candida overgrowth. I tried everything too. Personally I still think IBS is somehow caused by a fungal overgrowth that is systemic. The most effective thing for me has been a yeast probiotic called Saccharomyces Boulardi. It is a cannibal yeast that attacks all the other yeasts. It is also the yeast that is in Kombucha, along with a bacteria. I first bought capsules of it and still use them semi regularly. But I found that my home made Kombucha is the most effective for me. You can also cultivate your own Boulardi by growing it on pureed fruit. There is a youtube video on that. I use the fruit in smoothies. And I've recently come up with a faster method by using apple sauce, or blending canned fruit. It is now a hobby brewing Kobucha. But definitely an acquired taste since my home brew is not that good.

Good luck

Hi Phil , sorry I havent replied sooner its been a hectic time over Christmas for all the wrong reasons. Interesting that you are brewing Kombucha . My son brews it but unfortunately he lives in Singapore so I cant access any of his . I'm still having issues the same as before even weeks later and its still driving me crazy .

Hi jayBeeI’ve had bottom issues for years and they got a lot worse after my hysterectomy 12 years ago when I was 49 for a prolapse. I’ve been so terrified of getting another prolapse it’s caused me to have pelvic floor dysfunction which means I have to strain every day because I have increased muscle tone. I try and do this the way the help sheets tell you so that I brace from my abdomen. But even so, I still get a really sore bum. I also have rectal hypersensitivity and often feel as though there’s something in there. Even a tiny amount of wind feels like a sharp stabbing pressure. I’ve seen several consultants over the past four years especially, and they examine me and either can’t find anything or tell me I have a few small piles. I’ve tried chicocaine ointment and it helps a bit but not much. I’ve also tried rectogesic in despair even though it’s for anal fissures - this gave me a banging headache and no relief from the symptoms so I stopped. I take a sachet of movicol every night which helps me to stay regular but it’s all such a nuisance. I’ve been on estrogen only patches for 12 years. Sorry I can’t be any help but the sitz bath is the best treatment out of a bad lot for me.

Hi sorry I can't help with the pile issue although I do have one constantly it doesn't cause me to much bother. I do however suffer from thrush and have done for some years I feel your pain. I had a scope once and candida was even found in my gullet! I see a gynaecologists yearly as I had cervical cancer when I was 27 and pretty much everytime they do the colposcopy there's thrush present. It's really horrible I'm on hrt patches which they've upped to try and relive the symptoms mainly dryness, pain and bleeding after intercourse etc ( sorry tmi for the men on here) 😌 what treatment have they given you so far for the thrush? I had a long course of fluconazle when it was really bad that definitely helped. Have you tried the candida diet?

Hi Kazza thanks for your reply . Ive been given quite a few courses for thrush . Usually in previous years ive been using 4 pessories , 1 each night for 4 consecutive nights and this usually does the trick . However back in September I was given antibiotics as i'd done a thrush treatment and was still having issues with pain and burning . I was diagnosed as a possible U.T.I over the telephone . Then as that didnt work and thrush was definately present i used another canestan course but still the same. In october I was then seen and examined in person by a gp who said it was Thrush and piles but mainly the thrush causing the soreness and pain . I was then given Fluconazole to take for 3 times every 72 hours and 1 pessary of canesten to use on the first day . I was also swab tested for infection. Everything came back negative for all the other infections , although no urine sample was tested . The week before christmas I contacted the gps again ( another telephone consultation ) this time i had to send a urine sample of and she said in the meantime she would give me another course of antibiotics . I nly took one and felt so ill I couldnt take them again .I contacted the GP back the following day and she said to wait until the urines test result cam back for before taking anything else . It came back negative for an infection. So back to the Thrush diagnosis . I then was put back on Fluconazole and internal cream . Ended up with a carrier bag full of the stuff. 24 fluconazole tablets 3 to be taken at 72 hour intervals then 1 a week for 6 months . the cream was just a one off . Ive now finished my 72 hour lots and due to take another every thursday , and Im still the same . Not sure what to do now but she did say to book a face to face appointment after the new year if i needed to . Ive to give my new upped hrt dose at least 6 weeks ( another 2 to go ) but not sure if i can put up with this another 2 weeks . I now am thinking urinary incontinence too as i'm leaking from somewhere but not sure if its just dicharge or urine dribbling thats causing the burning from Vaginal atrophy . I'm having to wear pads all day and started using a barrier cream but thats not helping much . Its just so uncomfortable.

Hello, I have the exact same issues and my gp doesn't care either, it is a horrible situation to be in. I use lidocaine 4% for piles (can get on prescription) and hydrcotisone cream 1% for vulva (from chemist, asda etc) but these on relieve in the short term and having a warm bath helps me too but these do not stop the problems from coming back. I have asked my gp today for a stronger hydrocortisone to try, good luck JayBee

Good luck to you too Speedbuggy . Its so uncomfortable . Not sure if you've read my reply to Kazza above but that's where I am at the moment . I've tried lidocaine but that hasnt helped as its basically just being washed off or diluted with the excess watery discharge. Also tried the sitz baths too . But someone needs to get to the bottom of this ...literally !

I use Benepura Ointment for piles, this relieves the pain and shrinks them. Natural product, Yarrow. Also for thrush like symptoms, vaginal atrophy I use Replens every few days. These two things have improved things for me.

Hi thanks for your reply . I have heard of Replens but not yet tried it . I'm basically now taking Vitaflora ( gp approved ) to see if that will regulate the vaginal thrush symptoms but have only been taking it for 2 weeks . Also as they have upped my HRT from 50 to 75 I'm hoping that improves the vaginal atrophy side of things. I'm considering asking the gp for the estrogen cream too if it doesnt work .
