Hard time urinating and weak stream
The PSMA scan back in June 2022 did show some residual cancer in the left side of prostate gland but we were told that the radiation and
Zytiga
are still working and will eventually get rid of it. PSA keeps going down, currently at 0.07. He had a TURP one year ago when he was diagnosed with PCa.

ONCT-534 Earns FDA Fast Track Designation in R/R Metastatic CRPC
“The receipt of fast track designation for ONCT-534 supports our belief that patients with [metastatic] CRPC who relapse after treatment with androgen receptor pathway inhibitors [ARPIs] such as enzalutamide [Xtandi] or abiraterone [
Zytiga
], represent an important unmet medical need,” James Breitmeyer

Hello again.
This was very quickly followed by a course of Aberaterone {
Zytiga
} which I was able to tolerate for about 10 months. It had to be withdrawn due to high liver readings. A 37 session course of radiotherapy was prescribed in February/March 22.

My Turn to Ask a Question
In my case, this would mean coming off Xtandi and going onto
Zytiga
(abiraterone). Health Canada has not approved the use of Lutetium 177-PNT2002 for metastatic prostate cancer but has allowed its use in this study.

Zytiga discontinuation due to high ALT/AST
Hey all, my father has to stop taking
Zytiga
due to high liver enzymes. What is the next step after this? I am upset because he was responding very well and his PSA came down to 0.6 from 277. We wanted to use Chemo as a last resort. As always, thank you for any help and advice.

Docetaxil first, then Pluvicto or Pluvicto first
Zytiga
failed and Dr Milowsky, my MO at Lineberger Cancer Ctr recommends I go on chemotherapy first and if or when that fails, go to Pluvicto.

Another week of complete agony
The pain is most always in areas where I have bone mets, so I don't know if it is directly because of the mets, or maybe caused by Lupron and/or
Zytiga
.

Any hope?
In 2018 I started
Zytiga
and Lupron.
Zytiga
stopped working after three years. In December of 2021 I started Taxotere. It didn’t help. I had 25 treatments of radiation treatment for my pelvis area. Treatment stopped on 7/18/22. Now I’m dealing with the side effects.

Lupron causing pain?
Plus, since I started
Zytiga
on July 1st, the pain is worse now than it ever was on the previous medications, Erlearda first, and only 3 months of Xofigo. Getting quite frustrated.

stopped Orgovyx and Zytiga with prednisone after 9 months (post radiation ) due to concerns about heart and QOL . What’s next ?
Why does the addition of
Zytiga
to ADT slow progression to CRPC ? Thanks to all for your incredibly helpful sharing of experience and information.

More Evidence Linking ADT for PCa to Adverse Neurocognitive Effects -Meta-analysis shows increased risk of dementia, Parkinson's, depression
The risk was even higher in men treated with newer androgen receptor signaling inhibitors (ARSIs), but the association was not consistent across the ARSI class: increased risk with enzalutamide (Xtandi) and apalutamide (Erleada) but decreased risk with abiraterone (
Zytiga
).

Chemo "reset?"?
Hello all, I believe I saw it here a while back where it was stated that after castrate resistance has been reached and then moving onto taxotere (chemo), that in some cases the chemo somehow causes some sort of a "reset" to where drugs like Orgovyx, Xtandi,
Zytiga
, etc., begin to work again..

Change seems inevitable
Their team, which I think is terrific, added
Zytiga
in Feb 2019. Bottom line is that I seemed stable for about 3 years with fluctuating PSA from 7 to 11, stability verified with bone and CT scans.

Rezvilutamide: new anti-androgen is better than Casodex added to ADT for newly diagnosed high-met HSPC. But is field too crowded?
With
Zytiga
(abiraterone), Xtandi (enzalutamide), and Erleada (apalutamide) and docetaxel already approved for this purpose, I don't know whether Jiangsu Hengrui will be able to make significant inroads into the market, unless they are able to compete on price.

What is my best fallback if the
Zytiga
doesn't work? After the Lupron,
Zytiga
, and the radiation, should I do Taxotere (chemo)? All input appreciated.......
