Waiting for PSA Rise Post-Surgery Before Obtaining PSMA-PET– waiting too long can put some patients at risk, MedPage Today, July 6, 2023.
The primary endpoint is
PSA
recurrence-free survival based on
PSA
, which will answer whether using PSMA-PET as opposed to conventional imaging following
PSA
rise after radical prostatectomy can reduce the risk of recurrence as measured by a
PSA
rise.

PSMA Pylarify Test
Finally getting a PSMA Pylarify test after a
PSA
rise from 0.64 to 0.90 in 11 weeks. Should be interesting. No symptoms from the cancer so far. At the current
PSA
level it would appear there is an almost 90% chance of locating cancer somewhere.

My scans showed spread but......
They report on prolactin dependent prostate cancer which has no markers like a
PSA
and more die from it than testosterone dependent prostate cancer. Check them out.

Help deciding on treatment
PSA
5.5, PI-RADS 3 (or 4?), Decipher .55 PSMA PET – no spread. PET results reviewed and confirmed by MSK. I’m officially classified as unfavorable intermediate, although I’ve been told that I’m borderline with favorable intermediate based on only 1 sample of 4+3.

Side effects experienced once you add Zytiga to Orgovyx
It took two and half months after MRI and PSMA PET scan to get his biopsy...
PSA
went up another 7 points in two and a half months. The MO he met within was very neutral and said it was up to my husband.

testosterone, following cancer, treatment with stable PSA
PSA
stable at .006. Low Free testosterone. My problem is fatigue and lack of strength in spite of working out. I am considering testosterone supplementation to a low level, while monitoring
PSA
. The literature supports that this is relatively safe, although to me, it seems illogical.

Long survivors with ADT + Xtandi (Enzulatamide)
5 years after RPE and RT my
PSA
is rising again. I am advised to start ADT + Xtandi. Who are longest still on this combination without getting castration resistant?

Sharing My Good News
Following a PSMA my MO thought the largest tumor in my left acetabulum was the culprit so after several rises in my
PSA
over a year long periods, only that tumor was radiated. That returned my
PSA
to undetectable where it remained for about a year.

Heidelberg update
The
PSA
went down from, 0,7 to 0,1. He still is in the program until next year and has to fill out a questionnaire every 3 month, has
PSA
tests and blood work done. It looks like the government pays for this, because he was enrolled in this study. So far so good

Asking for best guess info
As I've written, my husband stopped all treatment this past July after Lupron/Xtandi were causing worsening SEs while
PSA
was doubling/tripling from undetectable. He's now receiving palliative care and just had his first comprehensive bloodwork since the MO's on June 30.

Predicting Response to Bipolar Androgen Therapy - A Blood Based Biomarker Approach
Professor Denmeade notes that from the TRANSFORMER trial and all studies with testosterone about 30% of patients respond with
PSA
going down which is typical of the response seen in prostate cancer.

Clinical Trial Dallas UT Southwestern
I am a low
PSA
emitter and Gleason 9 all 12 cores tested. The point I want to bring up is that she remarked that now she can see why I responded so well to Lu-177. I will post part of her reply to me. This is only FYI ..you can take it anyway you want.
