Thyroid hormone treatment intensity linked to increased risk of cardiovascular mortality
Just how long can this go on? Positively defining in terms of TSH alone. "… exogenous hyperthyroidism or hypothyroidism, which is having thyrotropin levels inappropriately below or above the normal range". And then going to to suggest it is the thyrotropin level which is the cause? Without having even

Tania Smith's take on our recent model paper
Since I'm certain that some of this conversation with Tania Smith will be converted to an article eventually in thyroid patients.ca I thought I would relay verbatim her current thoughts after reading it. When the article from her eventually surfaces I am sure it will spout damning flames about what is

The Starting Dose of Levothyroxine in Primary Hypothyroidism Treatment
This paper compared patients started on a calculated full replacement dose of levothyroxine against starting at 25 micrograms and incrementing every four weeks. Since this paper - and others - the idea of starting on full replacement dose has permeated the guidelines in several countries. Unfortunately

Suboptimal Thyroid Hormone Replacement is Associated with Worse Hospital Outcomes
Not saying this paper doesn't have its place. Of course it does, otherwise there is too much guesswork and assumption. But, I ask you, is anyone actually surprised by this? The cost of treating (in terms of length of stay and/or readmission) must be vastly more than providing adequate levothyroxine.

Reply to queries on paper by Bianco et al
This is a reply to a group querying the validity of Bianco et al's paper on T4/T3 prefrence by some patients. I thought it nteresting to read the various arguments so I put it in full here, as it's behind a paywall. Response to Letter to the Editor From Bonnema et al: “Comparative Effectiveness of Levothyroxine

Reference Levels for TSH in Iodine-Sufficient Low-Risk Pregnant Women
Just don't be different! If TSH levels are, as stated, population specific because of differences in ethnicity, then we need a rather more significant changes than just making the top of the TSH reference interval a bit higher. I think it easy to imagine a clinic in the north of India. They pick up

Glimpses of understanding
This recent paper does begin to tackle the problems of therapy with T4 only. It is a start in that it does recognise patients who need combination therapy and that past trials were inadequate to study this. Plus the prospect of a more controlled T3
dosing regimen. It's behind a paywall. However I

Paper discussing TSH as a moving target with age which affects its diagnostic significance
This paper discusses how TSH values can change within-person and the effect on diagnosis using TSH as the preferred diagnostic test.

Slowly but slowly they are catching up
This new paper discusses the power of FT4 testing against TSH and finds FT4 better. That's at least a small step forward in that it challenges TSH-is-all thinking in therapy. The authors haven't yet caught up as regards using FT3 measurement as well, but even a small step forward is good. Hennessey is

Papers questioning TSH as a diagnostic keep coming
A new Australian paper which examines the value of TSH testing and its relationship to best hypothyroidism detection. Don't agree with all of it, but the concept that FT4/3 measurements are better than TSH is well covered.

Paper showing which thyroid hormone assays are sensitive to biotin
This new paper is behind a paywall, but the abstract says all that is needed. It identifies those test manufacturers whose methods use biotin which disturbs thyroid function tests amongst others. This gives TUK posters/admin some ammunition to challenge incorrectand misleading results a) by not taking

Skipping thyroid dose makes me feel better
I've been on thyroxine for more than 15 years and always has problems with tiredness and mood. Recently I realised that the days where I felt great over the last few months (and probably for years but I didn't notice the pattern) was when I accidentally skipped my thyroxine dose. And I mean really great

Help with interpretation of my new blood work
Hi all I have Hashimoto's and recently been put on old formulation Euthyrox after years on NDTs, as my conversion problem has improved after I starting supplementing with selenium. Ten weeks ago, I was on 125 ug Euthryox per day and these were my results: S-TSH 0.09 uIU/mL L 0.35 - 4.94 S-FT3 3.5 pmol

Why thyroid function tests are in such a mess!
This is an excerpt from an Italian scientist replying to me on the reasons why the various tests are not properly controlled by the companies selling them: He describes the way the various groups don't communicate with each other and therefor have no interest in getting the tests harmonised:
