Osteonecrosis of jaw after tooth extraction while on Zometa

Has anyone had a tooth extraction while on Zometa? I know tooth extraction should be avoided due to risk of osteonecrosis of jaw, but I have had a filling and root canal in the wisdom tooth. If this fails then there will be no choice but to do an extraction and I am very scared of developing this rare complication. Various research I have read state the single biggist risk of developing bioposphonate related osteonecrosis of jaw is in fact tooth extraction.

I was told I'd have to come off the treatment before having dental work. My oncologist won't even let me see the hygeinist. X

So many differing opinions. My dentist said if I wanted I could get my teeth cleaned at the dentist and check up every 12 weeks to keep them in tip top condition as I am on these meds. I have been doing this since I started on the meds including danosumab. My oncologist said it was fine. Debra xxx😍

Also the hospital oncology insisted before I went on these meds I had a letter from dentist to say I had an up-to date check up and full clear X-rays on teeth and roots . It was Nov 2020 and emergency only due to Covid but my dentist opened up surgery for me as an emergency which I was so grateful for.Debra xxx

I have been on Zometa every three months since 2013. Last spring I cracked a tooth on a popcorn kernel. My dentist referred me to a periodontist in their practice. From there I was referred to an oral surgeon. The oral surgeon said no way would he pull the tooth. He said if it came to it that the tooth should be grind down and sent me back to periodontist. Periodontist did a root canal and the dentist capped it.

I have a cancer free friend that gets the Prolia injections and her dentist pulled a tooth last summer and she has had no issues.

Hi, I have actually got Osteonecrosis. At the time I was on Letrozole, Ibrance and Denosumab. I should have had 6 weeks off the Denosumab before and after the extraction. I wasn’t and a week after the extraction I was given a Denosumab jab that caused no end of infections and eventually Osteonecrosis. I had to have loose bone removed (didn’t hurt at all) and I still have a small piece of protruding jaw bone where the empty tooth socket is.Last December I had to have another extraction but this time I had it done correctly. I am actually on Everolimus and Exemestane now plus the monthly jab. I had six weeks off either side from Denosumab and all went well. For me it was the Denosumab injection that caused my jaw problem. I hope this helps and good luck.

Thanks all for the replies. I had seen dentist at time of starting Zometa and x- ray clear. Onc said to see hygienist and dentist every 6 months and I actually go to hygienist every 4 since my plan covers this. Flowerfairies 2 - sorry you got Osteonecrosis but glad to hear it was treatable. I am future tripping here - it may not come to extraction - just the fact that it is a wisdom tooth - which under normal circumstances would have just been extracted - limits options. Dentist did the filling but high risk of needing a root canal and finally extraction in the future. But I guess like everything else with MBC - I should not worry about something that may never happen

Your oncologist will hold the zometa until it is healed.

I went off if Zometa for 8 months while dealing with a broken tooth. My dentist is trying an”old school” rebuild of problem tooth then using Invisalign’s to pull it down from the gum. My oncologist didn’t want me to go back on Zometa until all this tooth process is resolved.

Can you go off Zometa for a few months before extraction and wait to go back on until it has healed?

She said if not for Zometa the course would be to just extract

I will ask Onc when I see him in April ... what happens if extraction needed
