My doc started me now on Everolimus 5mg and exemestane 25 mg.. Has any of you been on these. This is post letrozole and palbase stopped working.

Hi. I was on both after letrozole stopped working (Ibrance wasn't available in 2013). The Everolimus (aka Afinitor) /Exemestane combination worked well for me for two years. I had some side effects with the Everolimus, though - elevated liver enzymes and mouth sores.

After one year of 5mg of Everolimus daily, my oncologist increased to 7.5mg gradually (7.5 every third day, 5 the other days, then 7.5mg every 2 of 3 days, etc. ). I hear that the strongest dose is 10mg, but hardly anyone can tolerate it. 5mg seems to be the standard dose, but I've also heard that that the dose can be lowered to 2.5mg.

You have really given me some hope here.

My doc howvere said to increase it to 10 mg post a blood test post 10 days..

Let's see.

i hope your mother does well with the new treatment.

One more note (I left this detail out in my previous response)...after one month tolerating 5mg of Everolimus, my oconolgist increased the dose to 7.5mg daily. It was too difficult for me...many mouth sores and liver enzymes (ALT, AST, and ALP) were significantly elevated. I stopped the Everolimus completely for two weeks, while continuing Exemestane. When my liver enzymes returned to normal, she put me back on 5mg for a few months and then did the gradual increase of the 7.5mg dose. That being said, everyone is different, and your mother may respond well with less severe side effects.

She has started experiencing mouth sores after 5 days and they are just starting. She has been advised to do blood test post 10 days. Let's see.

It seems this could elevate blood sugar as well. And hence the blood test.
