After my second iron infusion my serum ferritin catapulted from 55 to 588! Unfortunately, it did not stop the “bleed through”. Horizant, which is supposed to last 24 hours only controls my RLS for about 16 to 18 hours and then I get symptom bleed through. I was hoping that at least I could have eliminated that much.

Take it at 5:00 pm because the bioavailability is greatly increased depending on the meal's fat content. You can also add a dose of gabapentin before it is due to run out.
It doesn't sound as though the infusion flopped: it raised your ferritin just as intended.
Unfortunately raising ferritin doesn't help everyone and some who have high ferritin without supplements or infusion still have RLS: I did until I discovered magnesium and eliminated my other triggers and thankfully my RLS symptoms. But I was lucky in that I'd been too scared to take antidepressants or DAs for RLS, which I'm sure made my recovery easier. I'm so sorry for anyone who has in the longer term been damaged by a well meant DA prescription.
I presume you're already following the standard advice on Horizant that Sue has repeated - to take it once daily with food at about 5 PM?
