I notice my night time only symptom of hot sensation in legs (inside the leg not external) is seldom mentioned on the classic symptoms of rls. Are there other possible diagnoses? Also has link been shown between lower back pain and rls symtoms?

hot legs
Most likely some form of neuropathy.. or meralgia.
Key classic features of RLS are:
Your symptoms start late evening or when you go to sleep
You have an irresistible urge to move
The your symptoms are reduced or removed by actually moving.
If you don’t have all these then it doesn’t sound like “normal “ RLS. A few people experience pain or tingling as well as the urge to move.
There is a link between nerve problems (neuropathy) and RLS, basically if you have neuropathy you are more likely to have RLS. I haven’t seen a decent explanation of why. The following Wikipedia article says:
Neurological conditions linked to RLS include Parkinson's disease, spinal cerebellar atrophy, spinal stenosis,[specify] lumbosacral radiculopathy and Charcot–Marie–Tooth disease type

The article is pretty clear and goes into more detail on symptoms which should hopefully help you decide.
Hi Enzeno, yes I have the same "hot leg" sensation too on many occasions. They usually feel like they a swelling or getting tight when this happens, but they don't actually swell. Lower back pain can happen from the many positions we find ourselves in to try and relieve the RLS and the continual muscle clenching which ends up as referred pain in the back. I have 2 monthly massage to help with this and it really helps.
