Why not get off a dopamine agonist altogether to avoid eventual augmentation and switch to gabapentin?

That's the question she's asking about!!!!

Magenta--+ if you are switching meds because of augmentation, then you should taper off the Pramipexol as soon as possible, while taking the Neurontin for the next step .

It will take a few weeks for that to work effectively, and you will have to 8ncrease the dose slowly until it becomes effective.

The withdrawals from the mirapex will affect how you are feeling, depending on how long you have been on it.

Good luck.

Whoops. I confused Neurontin with Neupro by mistake. I know better.

I augmented on mirapex it depends how bad your rls is I see that on this forum they tend to stay away from dopamines I have been on Ropinorole for years I take it at night and a neupro patch I have zero rls issues at all I’ve been on there meds for 10 plus years no problems at all

It's not that " on this forum they tend to stay away from dopamines" it is that the Mayo Clinic Updated Algorithm states that this is no longer the first line of treatment for RLS because of the danger of augmentation. You have been lucky that they work for you and you haven't augmented.

Yes as Madlegs1 said, reduce very slowly, usually by .125 every 2-3 weeks or so. And neurontin (gabapentin) won't work fully until you are off mirapex for several weeks so there is no need to increase it until then. At that point you can start increasing it by 100 mg every say 3 days until you find the dose that works for you. If you take more than 600 mg but less than 1300 mg, take 600 mg 2 hours before bedtime and the difference 4 hours before bedtime. If you take 1300 mg but less than 1900 mg take 600 mg 2 hours before bedtime, then 600 mg 4 hours before bedtime and the difference 6 hours before bedtime. The reason is that it is not as well absorbed above 600 mg.

Thanks I have been taking Prampexipol (Mirapex) .125 twice daily for last 20 years and starting to need more so was switched to Gabapentin(Neurontin) 900 mg once daily HS Dr didn’t say to taper Mirapex but I have been taking at least one per day so I can sleep. Feel tired hung over all day.
