Hi i would like some help i have had chronic a long long time im on siffrol and trammel used to work but you no the story i had to up the tab lets till i was taking more than i should I've tried tablets to many to write down i am 73 put up with no sleep walking the floor all night not been able to sit long.and so on, im going to try marijuana vaped i do hope it works. the doctor didn't no how much i should try in my tank something blue i cant find the script the strength was 5 /10 im sure thanks all have a good day

Hello.

I have some good news for you which should significantly help with your problem.

It appears that you have been taking sifrol for a long time and more significanlty have been increasing the dose.

This is the cause of your problem.

Sifrol (pramipexole) is a type of drug known as a dopamine agonist (DA). DAs are known to cause major complications.

The first to happen may be "loss of efficacy" and this is partly what's happened to you. When it does happen it is NOT advisable to increase the dose.

Increasing the dose leads to an even worse complication which is known as "dopaminergic augmentation". It very much appears as if you are suffering augmentation.

There is a solution for this.

Since the sifrol is causing the problem then you need to reduce the sifrol and possibly stop taking it.

The tramadol you're taking is not going to have any effect whilst you're suffering augmentation.

I'm afraid it's not easy to reduce/stop taking sifrol, so you may have a hard time ahead of you, but unless you do things will continue as they are or even get worse. Reducing the sifrol in itself will improve things for you.

It's not easy because when you do reduce a dose of a DA, any DA it causes withdrawal effects which can be so severe that some people give up trying. There are, however, ways of reducing these withdrawal effects.

You will also need to replace the sifrol with an alternative RLS medication. This should NOT be yet another DA, i.e. ropinirole or rotigotine as these cause the same problem.

The two alternatives are either an alpha 2 delta ligand or an opioid.

Anybody suffering augmentation should also have a blood test for serum ferritin. If the ferritin level is below 75 then it's a good idea to start taking an oral iron supplement. Iron deficiency is a causative factor in RLS.

It's possible that if you're taking any other medications they may be making your RLS worse, in which case you could consider changing them or stopping them.

In summary, sifrol is the cause of your problem, you need to reduce it or even stop it. If you want more information about reducing sifrol and/or what to replace it with, don't hesitate to ask.

Follow his advice & reduce the Sifrol very slowly over at least 3 months.

It’ll be very difficult but for the 2-3 weeks when it is at its worse after you stop completely- you can take tramadol every 4 hours.
