Applying for CHC

As I seem to be one of the few who has not got CHC I think it is about time I had another try as our money is fast disappearing. Any hints would be welcome. Would it be worth talking to Beacon. Although John can do nothing for himself I think they will say he needs caring not nursing. It us heartbreaking watching him deteriorate and not being able to converse he gets so frustrated . Pauline you should emphasis your worst day, the unpredictability that comes with psp it is a horrible illness, we should not need to fight to get CHC, I am sorry you are going through this, we have to fight there corner all the time, which is very tiring not just for you but your loved one as well. Fingers crossed that you get it this time, I am sure you will get a lot of help from these wonderful people on here. Sending you a big hug. George is not good at the moment, he fell out his wheelchair last Saturday, he is hallucinating some days, very frightening, and he has a syringe driver which is helping, but Sunday night had an hour sleep, sleep deprivation is the worst thing. Hi Pauline

I would read the Beacon material. It's very well written - clear and simple, but covers the ground well.

Do you have a Community Matron involved? Or, someone of that ilk who could be at the assessment stages to help give evidence? If not can you get your GP to refer you to one?

It is all about evidence. The Beacon advice documents are very good and very clear. I strongly suggest that you get a referral in for CHC (through your GP) and then read the Beacon material.

When you have done that I would download a copy of the Checklist (which is the first part of the assessment) and complete it yourself. This will help you organise your evidence.

It's not rocket science so don't get put off.

If you get past the Checklist stage the next thing is to download a copy of the DST form and complete that too. Keep these by you during the assessment and refer to them. They will help you give evidence without missing things.

Lastly - write down issues and problems as they are at their worst. Also include ways in which you struggle too. Though it is not actually in the 'rule book' if you are struggling they have to take it into account as they must ensure suitable care is in place.

Dear Pauline

We have very recently been awarded CHC

I followed everyones advice here and went through every domain thoroughly giving examples / evidence of the worst days.

I also kept a 24 hour diary for 4 days - when I typed it up it made such depressing reading - but what it showed was the level of care my husband needs.

We are going to be able to continue to use our existing care agency though they cannot put everything in place immediately I think they are worth waiting for.

It is a time consuming process and you must check

Everything someone else writes about your husband for accuracy. We had to wait for 2 months to get a definitive answer about the outcome which was hard. Now we just have to hold onto the funding as long as we can!!! Good luck I really hope you can get the help you need too

We involved the hospice and they did the third application for us and it has been awarded. Good luck.

Ben was never part of the interview process although the two CHC personnel who conducted the interview introduced themselves to him. The Parkinson's Nurse sat through the interview with me and added a few points as it progressed, our lovely Speech and Language Thetapist adde valuable notes to the form too. It took almost three hours and was gruelling but I managed to say all the things I had learned I needed to say from info on this site., Ben wax awarded it first time of trying thank goodness. Forgot yo mention that the Hospice Saff Nurse filled the form and submitted it after running it through with me, I had a chance to discuss and change a few points before it was submitted. xxx
