Unfavorable Intermediate PCa--treatment

Treated at UCSF. Chose radiation plus 4 months ADT (Orgovyx) due to concern of local meta. Dr. Shinohara performed tp biopsy and tp placement of markers. Alexander Gottschalk is my RO. Treatment: commencing 2 months after start of ADT, 2 fractions of Cyberknife to prostate. 25 fractions IMRT to pelvic area. Drove 60 miles each way for 27 consecutive treatments at Mt. Zion in SF. Moderate SE from ADT and radiation now 7 weeks post treatment. T slowly recovering. PSA now .04.

My advice: read Dr. Walsh's book (latest version). Find COE and best docs you can, and follow their advice. UCSF and Dr. Shinohara and Dr. Gottschalk, and their teams, are outstanding.

Just now saw your post and my husband also had his treatments at UCSF. His surgery was in 6/2022 but his PSAs have been inching up since Sept 2023. His latest PSA is .106 but they wont do anything until his PSA reached .2. What was your PSA before the salvage treatments? We are interested in hearing your experiences with your current teams, thanks.

I did not have surgery. I chose radiation with 4 months of Orgovyx.

I will start radiation soon and also doing hormone therapy. Fo you have any words of wisdom, any personal experience's you can share.

Get treatment at a center of excellence w the best doctors you can find there.

curious why you recommend his “new” book. I read the last one four years ago and it was all RP rah-rah, IIRC. Has he changed his tune?

I read the 2023 version and found it up to date and informative especially re the Decipher test etc. I also viewed many pcri YouTube videos.
