i have prostate cancer with gleson scale of 8. Trying to figure out if radiation or surgery is the best course of action. Can any body talk about their experiences with erectial issues and inconsonance. I am 58 and looking st the future.

Both have side effects and it isn't the proceedure that does the difference but the performing docs and the equipment used. There is no free lunch, but, with prostatectomy you get the check right on the spot, whereas with irradiation you sign a perpetual IOY. I had surgery, yet flew across Europe (and paid out of pocket) to get the best care available at a very specialized institution. Was Gleason 8 as well, upgraded to 9 post RP, so nerve sparing was out of the question. No problem with incontinence 5 years now. Good luck.

I will echo what justfor says about finding an excellent physician, center of excellence whichever you choose. My husband traveled to obtain mri guided sbrt, for example.

I had RP from a very experienced Urologist at Hopkins and after 60 -120 days suffered very little side effects of ED and incontinence. I was diagnosed as Gleason 5+5 later with biopsy a 5+4.
Some escaped so after 3.5 years PSA rose so went through salvage radiation at the end of 2023. Recovered from that and Ogrovyx and feeling and things are working great. So every 90 days for a few more years I will get PSA tested etc.
So I echo Justfor's recomendation. Decide your path but go to a great highly recommended institution and with a great Urologist or Radiation Oncologist .
FYI I've been a member for a long time and I've noticed that Radiation is becoming more popular than Surgery. I too wish to advise you that you make sure you see the best doctor in the best medical facility to treat your Pca. Full speed ahead...

I have spoken to my urologist, i have had consults with endocrinologist, the radiologist, , and the surgeon. I am being steered by sergeon and the radiologist to go with Proton or IMRT. I still have to get a PSMA scan and MRI. I will continue with this Monday. Been experiencing mild pain in the groin area. It is concerning to me but i am doing what they are instructing me. They are good doctors so i will forge a head.
John I second your observation. I attribute that to improved technology but I am still wanting to see the “beam me up, Scotty” approach to all cancers.
