Need advice. I had prostectomy in February of 2020, then radiation therapy a year later as PSA rose. Now PSA is 1.23 and the oncologist now is recommending beginning Lupron injections. Would welcome any thoughts or suggestions. Thanks.

Yes start the Lupron ASAP. Also perhaps look for a good second opinion Doc. I would suggest a PSMA PET scan as well so as to know where the PSA is coming from. There is a YOUtube video with a lady Dr. on Latest advance Prostate cancer treatments (2022 or 2023?). They give details and suggest that you be on ADT + a second testosterone blocker. They also suggest a DNA test to see if a PARP inhibitor would help

The secondary drugs can be expensive so I would start looking for clinical Trials that fit your situation. They are looking at the two and three combinations to keep you in the mCSPC and delay transition to mCRPC where options are more limited.

I am currently in the SPLASH trial with LU-177-PSMA-PNT2002 for FDA approvall of a drug similar to Pluvicto but less treatments. Another targeted treatment in underway in TAMARCK where a molecular Chemo shot is sent to the PC cell. Another trial starting is for a S. Korean diabetes drug that has shown signs of stopping the feeding conversion of the PC cell from CSPC to CRPC. The good Lord has filled the pipe line, we just need to last until the correct treatment is found for us!
