Blood and clots in urine 5 weeks after RP

My husband (51 years old) had a Robotic Prostatectomy about 5 weeks ago. Pathology showed Pt3a, bladder neck invasion, perineural invasion, non-focal EPE, negative margins, no positive lymph nodes. Last week at 4 weeks post op he began having on and off blood and blood clots in his urine. Previously, he had not had any blood in urine since about 3 days after surgery. Dr advised to stop pelvic floor exercises for a week. Blood/blood clots continued sporadically. Largest clots have been dime sized. Anyone else experience anything similar after RP?

I passed some clots post-surgery, but not that long afterward. I would certainly report this to the surgeon. But, if you're just passing clots and not red blood, you're likely okay. Still you should let the surgeon know. Maybe he overdid the pelvic floor exercises, as the doc suggested.

Thank you for the reply! Yes, we are hoping that is all it is…didn’t know if it was a common occurrence. Unfortunately, he does also have red blood with the clots. Not a huge quantity, but definitely present. He is supposed to touch base with surgeon again if it had not stopped after resting for a week, so we will see what they say. Thanks again!

Not after the first few days. But, it does take some time to heal up. Docs advice was probably good. Perhaps be a little more gentle when resuming the pelvic floor exercise.

Take heart. Three years post surgery and I'm doing just fine and glad I went this route. But the required patience can be frustrating.

Before catheter removal did he had a cystography (x-ray) for confirming the tightness of his anastomosis. If not, there is a possibility that the catheter was removed before the new tissue had sealed the joint (anastomosis). The red (fresh) blood can be an indicator of this. Brown coloured clots are not alarming. I would ask for an ultrasound to rule out any anastomosis leakage.

Thanks…he did have cystography at catheter removal appointment 4 days post op, all was good at that point!

I did not experience very much clotting at all after catheter removal. As others have said, always best to keep your doctor informed. I just passed four years since my surgery, robotic radical prostatectomy, and have follow up next week. Hoping for another good report of undetectable PSA. I still have stress incontinence which is quite annoying, and ED, but I guess that's to be expected. I am considering alternatives for the incontinence. Good luck! This is a great forum.
