What is "high volume" GL 6?

I was watching Dr. Klotz's video on AS. He mentioned that people with "high volume GL 6" should reconsider AS. What exactly is high volume GL 6? He never defined it.

Just as it says: high volume means big.

I have the same question, have watched Dr. Klotz and had exactly the same uncertainty. I have 8/12 systemic cores plus 4/4 targeted cores positive G6. (80%,30,30,30,20,20,20,15,10,10,10,5%). Am I "high volume", and am I a good candidate for AS?

Others can chime in. But that seems high volume. What did your urologist say?

Also, did you get any genetic testing done?

Urologist said it's high volume and suggested either AS (despite high volume), RP, or Radiation and referred me to RO and UO. Have met with (2) RO, both kind of said surgery is better for me due to young age and AUA score 20-25. One seemed to lean toward AS, the other toward surgery. However both RO were very much offering options, thoughts, and information and made it clear that it's my decision. No genetic testing done. Will be chatting with (2) UO in coming weeks.

Really could argue for any of the three it seems, we're still gathering info from Dr and online, not rushing to a decision.

Your young age should not drive you to surgery. This is a common misconception and too often pushed by surgeons. Equal outcomes with radiation or surgery but the side-effects short term, and possibly long-term are worse with surgery. Please read some of Tall_Allen's posts and articles on his website.

Just listened to podcast on Dr. Geo Espinosa's website on Active Surveillance . He interviewed a Dr. Aditya Bagrodia, he is a urologist at UCSD. He was asked to give his definition of "high volume". His answer was "more than 6,7,8 positive cores. More than 50% positive cores, and both sides."
