Systemic treatments kill the cancer no matter where it is. While it is hormone sensitive, Androgen Deprivation Therapy that reduces testosterone levels can work for years. If the cancer starts to grow with first line ADT, second line ADT drugs can hold it back. Chemotherapy attacks dividing cells of all kinds. Xofigo radiation therapy targets bone metastases. Lu-PSMA targets cancer that expreses PSMA.

The best treatment at the right time varies with each patient.

They can give you a medicine (like Zoladex, Lupron, etc.) that will stop all of your testosterone production. That will slow the cancer down and prevent bone pain. If the bone pain gets very bad, they can relieve the pain with radiation. If you have osteoporosis, a bisphosphonate can prevent fractures and spinal compression.
