After my booking appointment I was given a phone number to ring to book my dating scan. They just wanted my NHS number and my due date so they could book me in as close to 12 weeks as possible.

I personally got a letter through the post with my appointment time for my scan, the midwives should arrange it after your booking appointment x

This is based on my experience, hope it helps. If you have recently found out you are pregnant you need to book your first appointment with the midwife yourself which happens at around 8 weeks. Then they will book your dating scan at 12 weeks. They will then tell you when you are expected to have any following appointments and either book them or you need to call and book yourself. But the scans will be book by the midwife directly.
