You need to discuss this when you go for your pre-op. Depends on type of anesthetic and your surgeon.
There have been lots of posts about pred and hip and knee replacement ops. Put hip replacement into search box top right.
Most surgeons leave it to the anaesthetists who give you a quick burst of hydrocortisone during the op.
Do mention it to both surgeon and anaesthetist though. Most nurses at the pre op don’t know the answer.
The other thing ....no tapering in the period just before or just after.
I got through my knee replacement 8 weeks ago with just some extra hydrocortisone from the anaesthetist.

No, you remain on your current dose, inform your Surgeon you are on Pred & tell the Anaesthetist what dose you are on & for how long.
They will give you hydrocortisone cover in theatre & if required Post Operatively.

That is entirely up to your surgeon and anaesthetist. It isn't a good idea to continue reducing your dose in the run up to the op - much better to be well and stable on a dose. They may give you some in the infusion during the op and will keep a close eye on you while in hospital. Some people may have a bit of a flare - others don't.
Make sure it is recorded in your notes at the pre-op and if there are any questions on the nurse's part make sure they check with the doctors. And if a nurse panics because your white blood cell count is raised - remind them it is a normal response to being on corticosteroids and doesn't mean you have an infection. One lady nearly didn't get her op when she should because the only person in the team who knew that was the anaesthetist!!
