I offered to be a living kidney donor for my sister who is in end-stage renal failure. Determined to be perfect match, underwent ALL tests and cleared until pre-op results showed high ALP. No symptoms except the high blood levels and sent for a liver biopsy. Biopsy showed bilary tract injury with scattered non-necrotizing granulomas. Findings were non-specific but compatible with PBC. Consequently given a diagnosis of PBC, Stage 1 and denied ability to donate.
Does anyone have legitimate, responsible information regarding the possibility of still being a donor? Donation center with flexible parameters?

I'm sorry to read this. It really is a difficult time for all involved. I'm sorry that I don't know the answers to your questions. I do sympathise with your frustration that you have come through the decision to give a life-saving kidney to your sister only to be told that it can't happen. I hope that you find a solution or peace with the current decision. Please don't let this overshadow the great love you have shown and share with your sister. I am so sorry to hear about your sister. My sister in law had a kidney transplant last week, from her family, so I know what you must be going through.
They are very keen to ensure the donor is not at any risk so I suspect that the criteria for donors is stringent in all hospitals.
It must be a shock for you, dealing with your sister and now the news on PBC. I imagine that you will you will be prescribed Urso. It has been very effective at slowing down the effects of PBC on my case but has taken months.
I wish you and your sister well and hope that an alternative donor is quickly found.
