Well that got me wondering too quite some time ago too. I think for a start there has to be different factors. In PBC for eg majority of us are found to have a positive for the antibodies - anti-mitochondrial (AMAs). Not sure what is present in PSC.
In PSC though it seems that the bile ducts inflame and can be prone to become blocked whereas in PBC the bile ducts that are under attack seem to start disappearing.
I think symptons of fatigue and itching seem common as they do in liver conditions. Seems that it is a more 'man thing' to have PSC whereas PBC seems more a 'woman thing' (excuse the terminology).
I think like PBC too, PSC can lead to cirrhosis eventually if that is how it progresses.
The PBC is what I have. Yes it has to do with the Bile Duct.. But the Bile does not go thru the Liver to clean out all the bad stuff...
Hi, as far as I have understood it PSC attacks the somewhat bigger biletree "veins" in the liver, wheras PBC destroys the smaller ones. But when I write this I suddenly realise I really have no idea... PSC seems to be worse though, more aggressive and more often leading to liver cancer. Next week I am starting an on line liver course. Might know more soon. Interesting question!

PSC attacks the primary, large bile duct whereas PBC attacks the smaller bile duct. Men usually get PSC and usually have Ulcerative colitis as well. Women mainly get PBC. PSC usually has a more severe prognosis, though they give Urso to both patients. PSC is known to not respond to Urso as well as PBC and since is the main Bile Duct, damage is usually more severe more quickly. That is the extent of my knowledge!
