I have only read about PA today and after reading symptoms on the website I think I have PA. On my blood test results last taken in September 2024, some are high and out of range. Which ones should I look at in more detail to indicate if I have this condition & how do I request a PA test at my GP practice? I have had symptoms for 9 years. Thank you .

Usually a serum B12 test is done along with folate Intrinsic factor antibody test diagnoses around 40-60% of those with PA

Full iron tests including ferritin

Vitamins D

Thyroid function.

Lived function and kidney function

Don't take any B12 until after your test.

You say 'some high' but seeing the numbers would help.

Much of this is about changes rather than absolute values, so if you have access to previous results it can be helpful.

If your Haemoglobin is in the reference range then you're not anaemic. In PA, a shortage of Vitamin B12 will normally give rise to a Megaloblastic Anaemia. This results in your body making bigger red cells. This is indicated by an increase in the MCV. Bigger red cells have more haemoglobin in them, so an increase in the MCH is typically seen too.

If you have problems with iron absorption or stores then a reduction in MCV and MCH is usual.

Needless to say, if you're deficient in B12 and/or folate, and iron, then the changes might not be as spectacular.

An IFAb [intrinsic factor antibody test] will find about half of cases with PA. If however your B12 is normal, without treatment, then you're far less likely to find IFAb. Likewise, Parietal Cell Antibodies can be a good indicator, but the test is not as specific as IFAb, as we find them in lots of other conditions too.

Diagnosis of PA is not simple, and really requires a good, clued-up doctor, as well as blood test results.
