Consultant doesn’t feel it’s necessary to find cause of gastritis/gastropathy following endoscopy. Does this sound right?

Hi everyone

As per the subject heading I’m aware that (from past experience) I’m probably expecting too much. I do feel though that there MUST be other things that could have been tested for or mentioned.

Following my endoscopy the lab report states:-

A. D2 biopsy (4 biopsies)

B. Gastric fundus (multiple biopsies)

C. Gastric body (4 biopsies)

D. Gastric antrum (3 biopsies)

A. Normal duodenal mucosa

B. and C. Gastric mucosa with moderate active chronic inflammation and focal surface erosion. No granulomas are noted.

D. Gastric mucosa with severe reactive gastropathy. No other pathological features are noted.

No evidence of dysplasia or malignancy.

Immunostain for H pylori on “B” is negative.

At my follow up the consultant stated that the endoscopy/biopsies showed that I didn’t have celiac disease or H pylori infection. I asked what he thought was the cause of my stomach inflammation/injury and he said it didn’t matter because the treatment would still be Omeprazole which is what he prescribed along with anti sickness medication.

After me hinting at other causes he offered to test for IF and PC antibodies. I don’t yet have the results of these.

I genuinely don’t like the idea of taking Omeprazole indefinitely as I have no idea how much stomach acid I’ve got to start with.

Can anyone suggest any other tests I could request in order to ascertain the root cause of the damage to my stomach?

Thanks Sleepybunny. That’s really useful. The Gastropanel test is very interesting. If this is a readily available blood test I’m wondering why doctors don’t use it. I’ll continue to research the information you supplied. 😊

You must be feeling a little deflated.

I am at present undergoing tests

I had the blood testing which came back Neg which is common as blood testing is often inconclusive.

I had the endoscopy which showed a hiatus hernia and errosive gastritis.

I was refered for a MRN scan ( magnetic imagining ). This showed a large part of my colon to be inflamed. The MRN is done to look at the terminal ilietis of which is the area that absorbs vitamin B12 and a common pathology to find here is Crohns disease! I copied this from my gastologists consultans letter to myself and Gp.

Due to his findings I am going for a colonoscopy tomorrow so that they can get a closer look at the inflammation and the ilium !

Be insistent and push for further investigation.

I am on lansoprazole which i'm not happy about, will discuss with the consultant when i'm next seen.

Based on my experience, this seems to be pretty typical in the US. Every endoscopy I’ve ever had beginning when I was barely 30, has shown gastritis, inflammation. Biopsies are performed and a proton pump inhibitor recommended for long term use. I’m also not comfortable with long term use but my dr insists the drugs can help prevent damage and possibly cancer due Barrett’s esophagus . I am positive for IFAB so it should be that low stomach acid is to blame. Doesn’t make sense. to me.

I would request a iron panel w/ ferritin, serum gastrin, CBC (complete blood count), CMP (comprehensive metabolic panel) & vitamin D. It is wise to have these done as a base readings should your stomach condition not improve. B12D is manageable but it requires a doctor that understands the condition and there are very few that due. Requesting these tests help you advocate for yourself. Depending on the outcome of the test you may want to have a thyroid panel run.
