Advice reference to high b12 blood test results.

I have had high out of range blood results since 2018.

2018. Blue horizon 735. Range (569).

2020. B/H. 1311. ". "

2021. B/H. 1272. ". "

2020. NHS *. 1532. Range ( 200 - 900).

For quite sometime I have being having bad joint and muscle issues, it's became a problem now reference to walking. Could anyone give me advice about b12 active blood test.

Thank you for taking the time to read this, all advice very much appreciated.

As I understand it the active b12 test only counts the b12 hat csn actually be used by the body.

The serum b12 counts all the b12 in your blood

From what I have read, it's the active 1 that used by body, a normal B12 b/test is total, also mentioned high level could mean deficiency ?. I do have a few issues at the moment,

Thank you for your reply

Have you seen your GP regarding your joint and muscle issues? I would think that these are more likely to be caused by something else. What other issues are you experiencing?

Sorry my reply is late., Thank you for reading my post. Yes was referred to rheumatologist in 2018 all bloods tests ok. Due to issues I changed surgery's middle of 2019 I didn't see Dr. until the end of Oct 2019 & not seen 1 since, what with covid & lockdowns as you know, its phone conversations I spoke to different Dr to the 1 I first saw, he's reply was what do you expect at your Age.

I was given levothyroxine by 1st Dr but when I have asked about increase after paying for blood tests the answer no, even though now my t3 is lower in range, than before starting levo.

This forum is strictly speaking for people with absorption problems - a high B12 without supplementation would imply that what ever is going on for you isn't an absorption problem.

Avenues that you could explore:

a) reviewing diet to ensure that you aren't inadvertently consuming high levels of B12 - eg from energy drinks eg redbull contains ridiculous amounts of B12 in a single can.

b) kidney/liver problems that are causing B12 to build up.

c) evaluating the possibility of a functional B12 deficiency by looking at metabolites like MMA and homocysteine that will build up if there isn't enough B12 getting into your cells (but those would have to be done in a context that ruled out other causes, such as folate deficiency for homocysteine and kidney problems for MMA)

d) checking active B12 might be useful - ratio of active / serum is usually 1/5 so if it is out of that rough range then it could indicate a genetic problem - definitely off topic for this forum.

Hi. Ref to engery drinks NO never liked them.About the b12 levels my Dr & the nurse just say that the level is OK. When I mentioned it's been high for 3yrs to them it does matter. (not seen a Dr for 19 months)

Reason I posted about active b12 b/test, someone mentioned that it can actually mean a deficiency.

From reading on nhs Web they mention there are 2 b12 b/test but nhs only does total, due to the tingling in my legs, eye issue, tinnitus, no engery.

Your reply appreciated.

Forgot to mention think it was 2016 my B12 was low & had to have a course of injections. Now I am the opposite?

injections put a lot of B12 into your system and then it gradually gets removed over time but the time that it takes can vary. the average seems to be about 1 month for cyanocobalamin and 2 months for hydroxocobalamin but these are only averages - some retain hydroxo longer than cyano and in some cases it took over a year for levels to fall. You may well we be in this category but GPs tend to focus on averages and are totally unaware that there is so much variation. All you can really do is try to get them to listen but this reference might be usefulonlinelibrary.wiley.com/doi... - only access to the abstract- full article is behind paywall - but it does refer to one patient retain raised serum B12 levels for 4 years.

Personally I am not worried about the high b12 dr's, nurses aren't concerned, but when someone mentioned deficiency also from reading nhs website mentioned b12 b/test can be misleading due to it being total, hence you can significant amount but the body cannot use it, as mentioned I do have issues, also unable to see a Dr doesn't help, unfortunately we have & are in difficult times. After reading the article you forwarded, (thank you) also difficulty ref Dr. I possible wait to have nhs b12 test done if still high then I will get active 1 done.

I had elevated B12 for about 3 years & I feel it wasn’t being utilised at cellular level due to untreated hypothyroid state. I hadn’t taken supps & diet wasn’t B12 rich either. I did suffer nerve issues. As soon as stated taking thyroid meds, levels normalised.
