I think it's when you have a normal b12 level. But have many symptoms of b12 deficiency. A raised homosystein and MMA level. Others know much more about these blood tests It's a clinical diagnosis of a combination of things.
One being nothing else found and responses to b12 injections
It's where you have plenty of B12 in your blood, but not enough is getting to the right places in the cells. You test for MMA and homocysteine, two chemicals used up in reactions mediated by B12. In a functional deficiency one, or both, will be very high.
