I have been bothered by high level anxiety for months. It used to be only after injections (B12 seems to be quite stimulating). Lately it doesn't seem to matter when I get my injection, my anxiety level is always high during the day. Had a 24 hour urine test and my catecholamines were slightly above normal, particularly norepinephrine.
I've started taking gabapentin because I'm developing more neurological problems with my autonomic nervous system and it seems to alleviate those symptoms as well as the anxiety.

I just had that reaction myself after my most recent injection! Was going to write in to ask about this. 3 hours after my 4th weekly shot, I started feeling "speedy". My heartbeat and pulse rate were fine, I just felt speedy on the inside, if that makes any sense. It was very uncomfortable and then I started feeling very anxious for about 3 hours, it abated somewhat but then came back when I went to bed. Felt fine the next morning but a few hours later, I started feeling very anxious again. The anxiety was gone by the third day after the injection. I have never had this reaction before. I thought maybe my body was telling me I've had enough B12? The other factor was instead of a one ml shot of 1000 mcg of B12 (methylcobalamin), the naturopath only had a 2000 mcg dose so she gave me a half a ml shot. I got the same amount as usual only in a more concentrated dose. I wonder if this was the problem.
Sorry I don't have answers for you, maybe someone else with more experience than I can explain it to both of us!
I can commiserate with you as the anxiety was awful. Hopefully someone on this site has some insights for us.

So far what we both have in common re: anxiety, is an increase in B12 injections.
in your case, your sixth and last loading dose and when you switched to monthly rather than every 3 months.
In my case, I was getting shots every 10 days but doc (naturopath) suggested weekly since I was feeling fatigued before shot was due. Started weekly injections, all was fine until the 4th one when I had that anxiety.
I am going to wait 2 weeks for next inj. as I do not want to feel that horrible anxiety again. I'd rather feel fatigued. Also, I have supplemented in the past, before the anxiety episode happened, with spray methylcobalamin, in between shots.
Maybe try going longer between shots and finding a good sub lingual, spray or nasal spray for in between if your B12d symptoms come back.
That's all I can think it is. It's like the nerves are overstimulated (as_tracy_ points out in her reply) when the doses come too close together? Can't be the type of B12 as we are on different forms of B12.
Just a thought.
What form of B12 and are you taking any other medications or supplements?
I just have high anxiety every day, I am on citalopram to help relax me at night and have had CBT and it lessened but has come back. Most of the time I do not even know what I am anxious about

am getting hidroxo, monthly. I skipped this month since last time experince was traumatic for me. I have occasionally some anxiety but this was horrible. I take no meds or other supplements beside b12.
I got loading inj in February and I had this experience with my last, 6th loading inj. Then I got next two inj on 3 months with little anxiety and then dr switched to monthly when I started having very bad bouths of anxiety after the inj.
Just a quick question. When you skipped your monthly injection because of the anxiety, did you have anxiety when you got your next injection? Thanks!
I supposed to get the inj around the 10th of the month so I just missed the inj for this month. I am still thinking what to do. Maybe I go for the next one, I mean next month.
I understand. I don't want to go through that anxiety again and mine didn't even last as long as yours did! I am going to see if I can go a whole month rather than 2 weeks and supplement with spray methyl to get me through a whole month. The spray has been pretty effective for me while I was waiting for the next shot. Previous to my getting the 4, weekly injections, I got then every 10 days and never had an anxiety reaction. What a dilemma! I'll report back as to what happens when I next get another injection. Good luck to you!

I haven't been tested for pa but my mother had it. My serum B12 was 290 and my MCV was 96, so my GP said I did not gave pa but I also was iron deficient which makes blood cells smaller and he didn't factor that in. My MCV reading probably would have been over the top limit of 100 if I wasn't iron deficient. I also have bad IBS and it might be an absorption problem due to that.
The GP started me on Cyanocobalami n once a month but it did nothing for my low B12 symptoms, of which I had many. That's when I started going to the naturopath doc because she understands about B12. Switched to methyl
once every 10 days, was not quite getting me through & that's when I started on weekly injections. Sorry for this very long reply!
I will go back to my GP in January and request a PA Happy Holidays!

Wanted to let you know that I had another B12 methyl injection 5 days ago and did not have any anxiety whatsoever!
This was 2 weeks after the injection that made me so anxious.
And actually, I didn't feel as many benefits from this last shot which has happened once before. It's like the shot doesn't "take". Have you had this experience?

I was on Hydroxo injections before this and suffered anxiety for the first 2-3 weeks after injection also felt very very low

I find that 2/3 days after the injection my anxiety is huge, shoulders round my ears the lot, then a few days before my next injection is due, it starts again but literally lasting about two days each time. I have my injections every 4 weeks and have never been fazed by needles so it certainly isn't that. In fact I seem to be developing a real pattern which I must remember to record, which is obviously in place of doing exciting things like living.
I do hope your anxiety levels start to diminish, it is such a rotten feeling but if they don't, remember how many others understand that horror.
