I have been reading up in some of your old posts and you say here; 11 months ago, that you have PA 20 years now and inject once a month, see;

So you moving to twice a month (I assume its cyanocobalamin) can not do you any harm, and hopefully helps you! I read you have high folate serum levels so not a worry there. See how you go, is all I can think of. I inject twice a month (IM, mostly cyano, once every 2 months hydroxo), and it suits me very well,

Usually in US one gets cyanocobalamin, its a very stable (cheap) form of B12; that has as a maintenance doses once a month, in UK we get hydroxocobalamin B12 (is supposed to stay in the body longer, cosr more than cyanocobalamin) and the maintenance doses for hydroxo is once every 3 months (in UK, in other EU countries maintenance for the same hydroxocobalamin is once every 1-2 months). Some get methylcobalamin which is a mote natural form of B12, is more expensive and least stable, not licensed in UK.

I agree with Marre. Here is the USA it is cyanocobalamin. The cyano-B12 is easier and cheaper to produce than hydroxo-B12, methyl-B12 and adenosyl-B12. Cyano is also more stable as the basic B12 molecule has a real affinity to Cyanide. This is the reason that hydroxo-b12 is the antidote (10 ml injections) to cyanide poisoning.

All four version degrade in the presence of light, especially UV light. Hence they are all sold in dark bottles. It is also important to store them in the dark. The price is relative to volatility. Cyano is about $5 per mL and Hydroxo is around $100 per mL. I've no idea what methyl or adenosyl injections would cost. A small bottle of methyl sublinguals are about $25.

Currently, I self inject and get a 4x 1ml (strength 1000 micrograms per ml ) prescription per month. I use 1ml each week as I was having problems at 1 every 2 weeks.

As I have quite a stress fill commute and job, and about a year ago I noticed that the 1 ml wasn't lasting the whole week. I also supplement with methyl-B12 sublinguals and folic acid during the last portion of the week, and especially about 30 minutes before going into a meeting. Otherwise, I couldn't think clearly.

I figured that it was a problem of exponential decay of the B12 in the blood stream as the B12 diffuses from the muscle into the blood and the kidneys and liver filter it out. The more I inject, the more lose. So I compromised with my Dr and started injecting 0.5 ml on Sunday on the right and 0.5 ml on Wednesday on the left buttock. I can some times coax 0.6 ml from the vial which has a rubber stopper and is not an ampoule. My Dr wouldn't give me 8X per month.

I inject at night just before going to bed and sleep off any detoxification symptoms. Monday and Thursday I can be a bit irritable but I can really work. Tuesday and Friday I'm good and Wednesday and Saturday I am bit sluggish but no brain fog or any other symptoms.

I also try to exercise with walks at lunch and I swim for 20 minutes on Monday and Thursday evenings. This releases some B12 from the muscle into the blood stream so I'm good on Tuesday and Friday. As I tend to do physical work at home on Saturday, I'm also good on Sunday.

I take a multi-vitamin and B6 each evening and Folic Acid and TMG each morning. I keep a log of my injections and keep all my pills in a weekly dispenser (14 compartments) so that I can remember to take each. I keep a penny in the compartment when I should have the next injection. I discuss my symptoms with my Dr when he does the B12 test, which is now once a year and any time I visit to office. So he is happy with my progress.

This regime has levelled out the peaks and valleys and had been working for me for the past year. As long as the manufacturer keeps making the Cyano-B12 (which was stopped twice in the past 4 years). I had to go hunting for a pharmacy that still had stock. I also got 10x 1mL ampoules of cyano-B12 from Canada over the internet during the first shortage. (watch out as they sell dosage strength of 100 micrograms/mL instead of 1000 mcg/ml on the internet.) These don't work with this regime as, once the ampoule is cracked open, I have to inject the entire 1 mL.

If I get stressed, I notice that I go down faster. Also I'm gluten/casein intolerant and if I eat something wrong, I get tiredness and brain fog symptoms. Finally, I was also diagnosed with low testosterone so I have to apply androgel 5ml once or twice a week to my shoulders and stomach area maintain energy. This varies depending on how I feel and how itchy I get from the gel. If I apply these, I do it on Tuesday or Friday mornings after I shower, and I don't shower or change my T-shirt the next morning so that I have it on for two days. All my T-shirts then go into a special wash so that it doesn't come in contact with the clothes of my other family members. I also shower to wash it off before going swimming so this medication is more of a hassle than the B12 injections.
