I’ve posted quite a lot recently about my Severe Spinal Stenosis and Heart Failure.
I’ve now had a second opinion and, unfortunately, it is that there’s no further treatment suitable due to Heart Problems so I have now been taking Buprenorphine Patches for 4 weeks. The Pain Consultant started me off on 5 mg but they’ve given me no relief only continuous feeling of nausea.
Next week he wants me to increase to 10 mg and then 2 weeks later to go to 15 mg.
My question is:- should I still have nausea after this time and is it likely to be worse when I increase the dosage.

Not sure sir everybody's body chemistry is different I started on the 10 microgram patch and increased to the 15 a month later and it's the best pain control I've ever had much better than Ms cotton and oxycodone combined. I still take three 5 mg oxycodones daily for breakthrough pain but as far as for continuous pain relief the patches better answer to my prayers I have been in pain management for 9 years and just on the patches for the last 6 months thank goodness I found them
Thanks for your reply. I’m having to rely totally on the patch for pain relief as, apart from paracetamol, and due to my hear failure I can’t take any other pain relief.
Have you tried physical therapy yet? (All those drugs have long term side effects). Bowen therapy can help lots, or the later development NST. Thats my work till I retired! Will tell more if you want? Cheers TimbowPSP
I’ve been on them for the las 10 years started with 10 then 15 and for the last 5 years 30micro gram . They’re absolutely incredible and touch wood I have had no bad side effects .Change patch once a week so easy . Only problem I’ve had is in the last month my GP has changed the manufacturer from Butrans patches to a generic alternative which don’t stick as well especially when bathing.
When I started from 5mg, I was in a miserable amount of pain with my back/leg nerves and feeling on the verge of being sick literally all the time from like the 2nd day on them; but told I couldn’t have anything for them, every 4 weeks I was increased by 5mg up to 20mg. I’m in a lot less general pain now, getting comfy at night is easier, however, I still get breakthrough pain, the feeling of nausea is less now after a few months of the 20mg but still present especially if I fall asleep directly on the patch; it also really makes the head feel like it’s got wayyyyy too much blood up there and can’t get it out, so I find it’s a trade off. Your body does get a lot more use to them, but it’s a long waiting game and I feel for you!
I've been using Buprenorphine for going on 5 years and have been on various pain Management medications (Oxycodone, Oxycotin Hydrocodone, Morphine, etc.) for the last 17yrs. Knock on wood, no issues or side effects other than constipation. Regarding the Buprenorphine, best move I ever made for pain Management. Titrated up from 5mcg to 20mcg over 1yr and have been on 20mcg for 4 yrs. No nausea but certainly the best alternative to other options however, 5mg oxy is still required for flare ups. I’ve now increased to 10mg. Unfortunately I’ve had a terrible reaction to the increase. Yesterday and today I’ve had “episodes” of breathing problems, excessive sweating, nausea and almost “passing out”. I spoke first with the Pharmacist who said this respiratory distress could be caused by my Heart Failure.
After consulting the GP he advised that I remove the 10mcg patch immediately! He didn’t think I’d have any problems with withdrawal as I’ve only been on 5mcg for a short time.
He then offered, again, things like Pregabalin or Oramorph but I’ve decided, with his approval, to wait until my next appointment with my Pain Consultant.
