l have had pain for 60+ years, now it is continual, I need to be able at least to wash and feed myself! the latest pain (I had a fall last week) is concentrated in the sciatic nerve area, how can I ease it.

Sir, Have you visited a physio? Which area of your back hurts and if you can describe it a little more here maybe someone of us can help. Describe the pain, when it occurs and when it increases etc. Cheers
Physio is a good idea. Some are more helpful than others. I suggest asking your physician
Do you have a diagnosis? Arthritis? X-rays?
Alternating heat and cold every 20 minutes (not too long) can help calm things down if inflammation is a factor. Otherwise, try moist heat for 20 minutes at a time. When my sciatica flares up the pain is intense. It is very stressful. The increased stress and anxiety make the pain unbearable. I've found stress reduction very valuable. Can you do any activities you find calming? Try reading a good book, soothing music, meditation, etc.
When I am able to calm myself, the pain reduces enough so that I can do what really helps longer term, I move as much as I can. I've found walking helpful. I use forearm crutches for safety when the pain is unbearable. A walker or cane can also be helpful to avoid falls.
