My Mum talked about someone she knows who had a care manager, from social services, who looks after the care package as a whole. As my care for all my conditions is not being over viewed.

Normally when someone is referred to social services they are allocated a social worker and a needs assessment is carried out.
If there are a number of health professionals involved in a persons care a care planning meeting is held. From this a care plan is worked out, this is based on client needs. The plan is written up with named officers and if home care is to be provided who will provide that care, what level of care will be provided, number of times a day a carer will go in and length of time needed to provide that care.
Sometimes if a person is in need of a great deal of care, they can request direct payments. Social services assess the care you need and workout the cost, that money is banked and disabled person can then employ a PA/Care staff and pay their wages from that pot of money.
The downside to this system is; you become an employer and you have to pay sick pay and holidays, plus, employ someone to cover when the staff take time off.
If you require help within the home or addaptions you'll have to contact your local social services office and request a needs assessment.
Unless your in need of a high level of care they'll look at aids and adaptions that might help you within your home and in your daily life.
