hello I am moving from wales to Sussex in the next few months and I’m wondering how I go about getting onto the new hospital rheumatology list. Currently 3 months into adalimubab and seeing slight improvements so don’t want to come off this drug, concerned how I’ll get the injections delivered to me though. What do I do to transfer my care?

Once you know the GP surgery your moving too they will set up everything in sussex.Just make sure you have enough jabs to give you some over lap.

I moved back to West Sussex over 10 years ago and it went ok.a they have to keep treating you as your already diagnosed.

Your only complication is your moving from Wales to England .

You need to find out which GP practice you will join, they will make the arrangement with the Sussex rheumatology department . if you are in contact with anybody in the town you are moving to, they may be able to recommend a practice. But do be careful. I moved quite recently and thinking I was being terribly efficient. I spoke to the practice I was leaving and they asked me for my details & which practice I was joining, which I thought was perfectly reasonable, but they immediately sent all my details to the new practice, which I wasn’t moving to for a month (I had told them the date I was moving) and I ended up toing and froing to pick up prescriptions et cetera. Luckily it was only 5 miles but I think you ought to get it better organised than I did as you are moving a bit further than that.

Try explaining to the practice you’re leaving and see if they will give you enough medication to carry you through the first few weeks.

my consultant said all had to be set up by my GP and it was very easy and all sorted. The only thing I made sure I had done bloods and medication delivery sorted as close to move date in case it took a while to swap over. It was very smooth!

When I prepared to move my old consultant wrote direct to the new consultant he knew in the new area to refer me.

You can ask your local hospital PALS (Patient Advice and Liason Service ) to help with ensuring a smooth handover.

I moved to West Sussex from Warwickshire about 18 months ago. I signed up at a local GP practice immediately after moving, arranged an appointment with a GP who swiftly organised the needed referral to rheumatology. I was warned by my GP that the waiting list for new referrals was a year however Warwick kept me on their list, with ongoing monitoring and medication, until they knew I had an appointment here. I suggest speaking to your current rheumatologist to ensure they will provide continuity of care until you can be seen in West Sussex.
