I recently started on Abatacept weekly injection which is a T cell co-stimulator and not an Anti-TNF and have been told one week pause before my first booster and two full weeks pause after, but I did get zero vaccine antibodies last year whilst on Rtx so my consultants are really cautious with me this time. If you can’t get in touch with your rheumy to confirm what to do, have a look on Creaky Joints website as they have a section re pausing meds for the vaccines , but you may have to scroll down a bit to find it. Good luck.

Hi again, this is what’s on Creaky Joints Website for Humira and vaccines.

“There isn’t a clear answer about holding most biologics, including TNF inhibitors and interleukin inhibitorsMany people have questions about whether to stop taking biologics that are not included in the above list, and unfortunately, there are no definite rules. The ACR guidance notes that its task force was mixed about whether to advise for or against holding the following biologics at the time of the third dose:TNF biologics (ex: Cimzia, Enbrel, Humira, Remicade, Simponi and Simponi Aria)IL-6 biologics (ex: Actemra and Kevzara)IL-1 biologics (ex: Kineret, Ilaris)IL-17 biologics (ex: Cosentyx, Taltz)IL-23 biologics (ex: Skyrizi, Tremfya)IL-12/23 biologics (ex: Stelara)Data suggests that people on these medications do not have a dramatically reduced response to the COVID-19 vaccine. Some doctors may recommend you temporarily hold them out of an abundance of caution; maybe there’s a chance doing so could increase your response slightly. Others may recommend that you continue to take them because stopping them could impact your risk of a flare and there is unlikely to be a big impact on vaccine response. It’s a personal decision that you and your doctor can discuss together.“

I'm on Humira.

My team have always given me the same advice (I checked for the first few jabs) - evenly space things.

I inject on a Friday so Humira one Friday, jab (Flu/Covid) following Friday, back to Humira the next Friday.

I haven't asked if they would advise anything different for having Flu/Covid together as I don't intend to do that. I am fairly unwell after the Covid jabs so want to keep on with the separately.

Each area seems to have different thoughts.

I have been advised to continue my Benepali but hold my MTX for 2 weeks after the vaccine. I have previously checked my antibodies having taken my benepali, MTX, prednisolone and hydrocortisone. I tested as having a strong response although I scraped through with a score of 5, strong response was 5-10.

I was on Humira at the time of my third vaccine and was told to take it as normal. It was only methotrexate they wanted to delay, but I was only just starting it. Advice is changing all the time though and might depend on the person and how controlled their disease is, so I would ask your team.

General advice, not just for vaccines, seems to be not to stop medication unless advised to.
