Looking for previous post!

I was reading a thread yesterday and someone was saying that they keep a record of results/symptoms etc etc on a piece of A4 paper - I'd like to replicate that, but can't find the post - doh! Can anyone help please?

Thankyou, I did find it. I’ve copied your idea & im sure it will be very useful. I’ve been in considerable pain for a while & want to keep a record to help me decide whether to try a diff med.

As said I've found keeping the journal so valuable, its difficult to overstate it and due to all my submissions ( 1 A4 bullet pointed letter) of summaries at each appointment, I've got a written track record in my medical notes, which counter balances a lot of errors and misdiagnoses. My care care has improved and my consultant voices she is more tha happy to receive the summaries. Good luck with your pain management.
