Care Quality Commission inspection of Norfolk Community Health and Care NHS Trust
The Care Quality Commission will be inspecting this Community Health Service from the 15th of September 2014. Services include things such as adult respite care, podiatry, sexual health services, community nursing support, dental services, community hospitals (such as Dereham), health visiting, palliative care and physiotherapy.
The inspection team is very interested to hear from anyone who has used services in the last 12 months about their experiences. The inspection team is also keen to hear evidence from local voluntary and community groups about the quality and safety of care that their members and service users have experienced. Please tell us about your experiences by Monday 1st September 2014.

Please put the name of the service you are sending information about in the title to help us pass it to the right inspection team.
You can send evidence of high-quality care as well as any concerns you have identified. We will use your information to help the inspection team plan the inspection and what to look for on the inspection. The inspection team will ask these five questions about the trust and its services:

• Is it effective?
• Is it caring?
• Is it responsive to people’s needs?
• Is it well-led?
