I had a transplant about 10 years ago. So far so good. God sent. But, I had a third gout (last one was 6 years ago) about 9 days ago after the transplant partially due to Cyclosporine. I took Colchicine to treat the flare this time on the 7th day as symptoms got worse on the 5th day. After 0.6 mg Colchicine dose for two days, the symptom is getting better, no pain. The Nephrologist suggested that I take Allopurinol with 100mg/daily after completing the Colchicine for 3-5 days once the symptoms improved. My UA is around 7 to 8 mg/dL for the last 10 years with reasonable diet control (no high purine food). My Primary Dr suggested that I further control my diet. My HLA-B is not HLA-B5801 (no single match), so the risk of taking Allopurinol appears small.
These are potential four options for me
1) Allopurinol as suggested by Nephrologist and monitoring the UA in month or so
2) Strict control of diet as suggested by Primary Dr. I may be able to lower it to below 7 mg/dL. Not practically to 6
3) Switch Cyclosporine to Tacrolimus
4) Use Febuxostat instead of Allopurinol
Which one should I take? Any advice will be really appreciated.
