TLC: Transforming the Gift of Life into a Transplant Living Community (TLC)

We would like to invite you to become a valued participant with our patient to patient engagement project! The Patient-Centered Outcomes Research Institute (PCORI) supports engaging caregivers and families in research that matters to them. The purpose of this survey is to gather information from everyone touched by the transplant experience through all its phases. The questions cover gaps in patient education related to organ transplant. Our goal is to create patient/family education to address these gaps to help future patients set real expectations before a transplant and prepare for post-transplant life. We need many viewpoints and experiences to make this a true reflection of the transplant community. The survey does not capture any personal identifiers so you may feel confident that your privacy is respected.
