We are super excited to be working with the NKFM to build a Personal Action Toward Health (PATH) specifically designed for Hemodialysis patients needing Skilled Nursing, Physical & Occupational therapy and/or Long term care. Our goal will be to host these meeting each week to help inform and empower those in our care. More to come soon!
