Medication question: Dapagliflozin (SGLt2). I think this is also called farxiga and other names

Hi everyone,

hope you all are well. I am doing alright since my first diagnosis back in Nov 2022.

My last Creat(about a month agao) was 2.39 with EGFR at 36. Pretty much the same.

I was wondering if any one is taking Dapagliflozin. My Neph recommended it and says might help in proteninuria.

I read it may cause UTI and what not but i see its primarily for patients with diabetes. I am NOT at all diabetic.

has anyone with CKD stage 3 been taking this class of drug and found it to be useful during their course? any improvements noticed in blood or urine work.

I cant help but thinking of what if it adds more problems. I dont want to add anything new to this condition here.

i read somewhere it is not recommended or contraidicated for people with eGFR less than 30 mL/min/1.73 m2.

Hi Kanna, My husband went on Farxiga last year. At the time he was GFR in the 30's. He is now at 59. This was the best thing for him. He also does have diabetes, and the Farxiga has helped to stabilize those numbers also. The yeast thing is something to consider, but.... check how many medications say that is an issue. If you are careful about hygiene, it won't be an issue. My husband has had no issues at all. Just like anything, what works for one may not work for another.

Don’t forget that yeast overgrowth is a susceptibility and weakness based on past history, medications, and anatomy, among other factors. It isn’t as simple as just hygiene—I wish it were! Terrific that your hubby is doing so well on his med though—not to be overlooked :-)

As for the topic, I finally agreed to try ozempic for my blood sugar values and it worked to lower them significantly. Unfortunately, it also gave me constant diarrhea and that was that. After 6 weeks, I felt it time to pull the plug.

Oh too bad. And sad because for many people it has the opposite effect of blocking you up. I suffer from terrible constipation until I figured out a regiment that works pretty successfully without hurting my kidneys. But Ozempic has done such a great job on my glucose.

And you are correct, some people are predisposed to yeast infections and it often is hormonal or chemistry.

i am ckd stage 4 not diabetic with gfr around 20. Last year tried Farxiga and I was extremely fatigued. I couldn’t stay awake for more than 3 hours before needing a nap. So I got off it.

Now on Spirinolactone and so far no side effects.

I should say I do have high bp due to my ckd and the new med helps control that., Also on losartan.

I am on Farxiga for proteinuria and to preserve my kidney function, and it has helped immensely, and lowered my protein to zero; no side effects, although I have seen a few people have had a few. The box warnings are scary, but those side effects are very rare.

I do get UTIs but not from Farxiga, but because I have to self-catheterize due to bladder cancer issues, these pre-date the use of Farxiga and are just part of the process it seems, I have an open script for Fosfomycin (Monural) because it is so common amongst cath patients. UTIs are NOT a hygiene issue, and neither is candidiasis (yeast).

I'm happy with it.

Edited to say I am not diabetic either!

How long it took you to get your Protein to zero with Farxiga ?

hi there

I am Stage 3b not diabetic, I do have high blood pressure, I was at EGFR 35 on my last bloods, I am due for another visit to renal department next month so I will let you know if there’s an improvement.

When I went on Dapaglifloxin to help proteninuria, it also lowered my blood pressure too much so I’ve had my blood pressure meds tweaked. I’ve had no known side effects from the drug but will see if there’s an improvement on my EGFR next month

Has anyone been prescribed Empagliflozin (Jardiance) for ckd 3-4? Any comments?

I know a few people who have been on it since the beginning of the year, and they have had good results. I don't know if anyone on this forum is on it, but I post on a few other CKD support groups and the word is, it's been going well for most folks who have tried it so far, of course everyone is different.

Gave me six months of yeast infections completely drug resistant and unable to cope. Seems to be a weakness of mine. Hope it helps you if you try it :-)

I was on farxiga for about a month, but I couldn't tolerate it. it felt like my throat was closing up on me. I'm now on Jardiance since January and Trulicity for a month. Sort of on Trulicity. I had labs last week and my proteinuria went down but it's still way high. She dropped the amolodine and added Aldactone. I get labs in 3 weeks and see her in July.

Wow, so sorry that happened to you! Must have been scary. I hope the other meds can get your protein sorted, well as much as possible. 😊

Hello kanna . I was put on dapaglifoxin a couple of mmonths ago. I am pretty pleased with it. Too early to say how its affected my numbers due to other complications arising lately but it reduced my swollen ankles to normal almost over night. I have had no side effects and I think have felt better for it. Can't say the same for most of the meds I'm on!!!

I began Farxiga before I was dx with diabetes. My GP recommended it and I have had no trouble. Wish it had helped my swollen ankles!

Hi. My eGFR had been in the 20s and then increased to 40 (perhaps because my Lisinopril was lowered from 20 mg to 5 mg - not sure if that is why my eGFR increased). At that point my Nephrologist prescribed Farxiga and I have been taking it about 6 months. My current eGFR is 39. My Nephrologist says it is likely helping to keep me stable and that it is slowing the progression of my disease process, but my eGFR has not increased as a result of the medicine. I think I have had no side effects. I did have athlete's foot while I was on it, but I have had that in the past occasionally and it went away with treatment.

Jardiance is recommended specifically for eGFR lower than 30 and Farxia for eGFR above (Jardiance just recently received FDA approval)

similar situation here. I’m also on farxiga and do not have diabetes. My protein numbers in my urine have been the lowest ever since I started this med almost two years ago.

I start for Farxiga in 2 months we will have to see because I am prone to a lot of UTIs as well as stones and I am at a 43 eGFR. My BUN, Creatine, and Potassium have always been really high, but on my last saturation test everything was in the green. However since I did drop from 44 to 43 on my eGFR, my doctor wants to keep me on some kind of medication to try and keep me steady so I don't lose any more points. I will definitely let you know if I get any of the side effects.

Farxiga acts differently on every individual. I see some positive comments about it, but for me, taking it for 7 months has increased my Craatinine level from 90 to 121 in 7 months. My Nephro stopped it and changed it to Metformin 500 mg, 3x a day. After exactly a month, lo and behold..my Creatinine was down to 64!!! Praise God🙏
