My gfr today is at an all time low of 25. Over the past almost 3 months I read Lee Hull's book, watched all of James' videos dadvicetv.com and have implemented all of their advice in order to increase my gfr which was 26 at the time. At the same time I started walking casually a mile about 5 times a week, I cut out all carbs and sugar and lost 24lbs. My A1c was 6.9 and today it's 5.6. Please see other labs attached. Now what?

How frustrating. Remember the GFR will fluctuate and it not uncommon. It went up one point on one of the labs and yes it is down, but only two points. At the time of the draw, it could be as simple as it was earlier in the day and you were more dehydrated. I would not panic. I also suggest that you see a renal dietician who can help you plan a diet or lifestyle that takes in all the medical conditions and not go off a book. It can make a difference. But in all, you are doing fabulous. Keeping the sugar down is really important. Do other draw and try doing it later when you have had more time to have water.
Frankly and simply it appears you are doing everything possible to treat or improve your situation and doing well. IMHO your labs look amazing. Small fluctuations are expected. Don't panic. Remember they will change and likely continue downward. Prepare yourself. Counsel with doctors, join support groups, see a counselor if you need.
You ask "now what?" My response is now continue on and go live your life. Cultivates hobbies, if you love music go to some concerts, travel, write or do things that bring you joy. In essence live your life while controlling diet and lifestyle.
Consult with your doctor's and get a referral for a renal dietitian. Check out sites like PKD, AAKP, NKF, AKF, kidneyschool.org to expand your knowledge and education about kidneys, your options etc.
CONGRATULATIONS on a job well done. Polycystic Kidney Disease has a life of its own, developing at its own pace. It sounds like you've been proactive in controlling what you can for your health and life. Well done. Keep it up.
