Yay! You've seen someone a) sensible and b) who seems to give a shite! Make sure the respiratory team review you - medical problems on pregnancy should be felt with by medics! Being admitted under obstetrics is usually the right way - getting a medical review of any patient (obstetric, surgical, psych, etc') is easy... Doesn't work as efficiently the other way round though!

Hope you feel better soon xxx

I've Got my own room so been upgraded from one star to two star ;) ... she is fab my consultant said the respiratory team wouldn't be able to see me till tmw but she wanted me in to start the steriods I'm so glad knowing that I felt so well I want to be able to leave here feeling well :)

Lol consultant just came in she said the steriods are being written up and start them today and drs and asthma nurse will be in tmw she didn't mention the rest? Actually she didn't mention t the nebulisers either I was hoping for them again!

Now a med registra came in and said he was not sure about steriods so I wouldn't get them today..I have to wait to see asthma drs tmw he doesn't want to make the decision as the effects on baby. .. but I thought decision had been made! !!!! Surely he can't override the consultant feel like screaming! Arrrrggggghhhh

No correction another reg said give them so on salbutamol.neb 5mg prednisone and a steroid neb so fingers crossed xx

Just 5mg of pred??? That's fairly pointless... The usual dose is 40mg a day for 5 days... Doesn't need to be weAned,..

in reply to DrFluffy

Is that the same being pregnant? Maybe The asthma drs will increase? Or Because of the steriod neb? Don't Know u never get anywhere too many cooks and all that! Maybe Get some answers today :)

Steroid nebs aren't usually part of the acute management of asthma. In the hospitals I've worked at, an asthma flair in pregnancy usually comes to the medics (and it's always a massive pain in the bottom to get an obs review and foetal well being check, but that's another matter!). Whenever I've instigated treatment in a patient I've admitted I've always started full dose oral steroids, and this has never been changed by a resp or obs team. Poorly controlled asthma is much more of a threat to you and your pregnancy than 5 days of pred... The only 'usual' drug I wouldn't be reaching for is theophylline - that's a nightmare to get right in non-pregnant people; but I'm sure if an attack was severe enough they would use it...

What have your oxygen sats (the % number from the probe they put on your finger) been like? Have you needed an arterial blood gas (needle into artery - usually st the wrist - to get a formal quantification of the amount of oxygen and carbon dioxide in your blood, and thus an indication of how well your lungs are coping)? Are you on a peak flow chart?

Glad you're getting better care this time. Hope the resp team sort you out. Xxx

in reply to DrFluffy

Yeah had the blood gas done last fri was normal oxygen sats have ranged

from 94%~98% which is good more the 97 since I've been on nebuliser and peak flow is around 380 which they say is good. .it was 350 which apparently is still good. ..is just the wheezing tight chest breathlessness and constant coughing if I'm not on nebs... bit they don't last...I had to use my inhaler this morning at 6 because I dint have nebs till 8...

but thanks dr fluffy u have given me more info t than anyone! !! X also midwife was saying medical won't touch me because I'm pregnant but they don't know about asthma! So Hopefully the respiratory team that are coming today will make final decisions! X also don't think baby likes the nebs his heart rate becomes bit erratic!

Thought I should check treatment guidelines for my own sanity! You treat acute asthma the same in pregnancy as non-pregnant...

Hey Babymother, hope your feeling a bit better this morning :) Xx

Thanks yes hopefully today as the respiratory team should make the decisions! !! So fingers crossed x

Thank you not breathless today horray! :)

Respiratory drs said continue on oral steriods and send me home on inhalers haven't got a wheeze and sats are good... but now the ob reg said I don't look great and wants me to stay in! And She is the one that sent me home on tues... lol can't win but I said I'd like to go home so she said ok were see how u are after lunch so wait and see :)

DrFluffy in reply to

It's good that the resp team are happy that things are settling - the obs reg is probably feeling a bit guilty: i'm sure it doesn't look great to your bosses when they have to organise an emergency admission of a patient who you sent home two days before :-p

Make sure you are feeling better before leaving though - best to feel on good form to reduce the odds of you ping ponging straight back. Did they increase your steroids to a real dose? Xx

in reply to DrFluffy

I just asked and it was 40mg bit when I saw the box yesterday it said 5mg...I'll check the tablet when it comes lol I'm coughing lots today dint know why! Everyone Wearing purfume is setting me off! The asthma nurse thinks I should stay in cos my symptoms are still apparent. . Just A bit undecided think I'll have a shower and a sleep and see how I feel after lunch.... how long does the Oak steriod take to work? Due My 2nd dose at 2pm x

Steroids have two main modes of action - one is fairly rapid, while the other works by altering the messages copied from your DNA. I usually see a marked improvement in acute asthma and COPD in 2-3 days, although this is personal experience not fact and the improvement is usually a mix if regular nebs and steroids xx

The resp team (they were young men) were like well ur sats and peak flow is fine and u haven't got a wheeze so that's fine go home...I said but I am struggling they said yeah well that's how it is!sort of attitude I felt like saying well there is a medical book and there is up patient u need to look at both! the asthma nurse said she will get me booked in to see the asthma consultant as my symptoms need to be managed.... and that's what is the most important thing as it's exhausting me... but the medical side shows that I'm not in danger. I've stopped coughing now .. so let's see howi get on with the steroids Thanks again dr fluffyu have been a star wish u worked at my hosp! You Sound like a bloody good Dr x

And sitting here listening to a woman screaming thru her contractions bless her. .. heard another woman screaming last nite and then heard baby crying was lovely. .. poor midwives one went to triage cos they were so busy so the midwife was on her own with lady on the toilet with baby's head protruding! Not long for the ladies near the end exciting times! Bless us all xxxx

My Mum and I chatted about her 5 births yesterday and they all different, she spoke of a high pain threshold....but then said....or maybe they just weren't that painful. She said she 'breathed' my middle sister out, no pushing. Her advice was breathe, focus and do not panic! Birthing is imminent now at 38+2, I had a wobble on last week but at then end of the day....baby has to come out and I have an open birth plan - take what I need and do what's right for both of us :) X

Think that's the best attitude to have! Wow Breathed her out wish I could do that! Def The thing is try and stay in control dont let the pain get out of

control! won't be long before we hear ur experience :) but excited bit scared is the unknown but ur be more excited xxx
