advise on how to manage the derma (skin) with DM

I've been told that skin rash/ulcers are the most difficult to treat . Has anyone had these issues and if so, how much time are we talking before improvement? Any medications that worked better than others? Azathioprine (imuran) made my skin worse, just an fyi for others. I'm already on hydroxychloroquine and I've read sometimes they add mexthotrexate or IVIG (if greater than 10% body surface area is affected). Anyone been on either of these meds and what sort of results? any adverse effects? My rheumatologist prescribed tacrolimus cream but advised it would burn- I've also got the heliotropic rash which frequently flares as well and am afraid to use the cream on it as it can be very uncomfortable when it flares - I have yet to figure out if there are any triggers. I've also been fortunate? to have developed ulcers on my fingers, inner thigh and just under my axilla. The ulcers on my thighs and arms took about a month to heal- one healed up and then the other one started. My finger ulcers developed 1-2 months after diagnosis.... it's now over 4+ months and no significant improvement. I would appreciate hearing from anyone that has had similar issues and what worked for you. As we all know research studies are limited so any I look forward to a response.

Thanks for the response. I hadn't heard of using cyclosporin... My rheumatologist told me methotrexate can contribute to interstitial lung disease and since I have the anti MDA4 antibody, it would not be a good option for me. I think I have been impatient and I am wondering if the meds are finally starting to kick in. No new skin ulcers in 30 days!

I wish you all the best and hope you get pregnant soon.
