My mother has a skin problem for last 4 years but there is no particular diagnosis. different doctors say different things. The affected area is itchy and scaly, and feels hard on touch too. please help.

It looks to me as though the skin is simply dry and non elastic. It does not look like psoriasis or an allergic condition. I would apply daily some Bio oil ,or a cheaper equivalent of light liquid paraffin.
Has your mum been referred to a dermatologist? In the meantime try a good emollient that suits her skin as suggested by xavier..... I have come to prefer Doublebase gel but there are lots of variations on the theme such as E45, Aveeno, Diprobase, Oilatum and so on that are available from pharmacy counters in big chemists.
Might be eczema, I have that and the symptoms your describing are a lot like mine but that looks more sever. When itchy take an antihistamine and apply something cool to the affected area to stop the itching. But a more permanent solution its creams like dermol, e45 or even a steroid cream prescribed by a doctor. If it is eczema the key for relief of symptoms is constant moisturising because with eczema the skin is dry, flakey with little elasticity. Hope it gets better soon.
Is your mum sedentary. I have seen this before on people that don't move much. I suggest the same as above, lose weight ,if possible and walk a bit more. I hope you don't think I,m being rude,I,m not trying to be just giving advise as I see it.
I think this is a very long job and I think it's due to very dry skin and not moving too much.
Yes, I agree, it looks like it might be caused by poor blood circulation to the extremities due to both just being an older person and not moving around so much; rather than an actual skin complaint like eczema. In the long run, more exercise is probably going to help. To improve circulation there are electronic products available. Also a low dose of ginko bilboa can have surprisingly good effects for some people (helps me with my cold fingers and toes). You can buy tablets very cheaply in e.g. Wilkinson. Obviously, not smoking and eating less sugar will also help with circulation, if applicable to your mum.
