I tried Staysure yesterday for a short Norwegian Fjord cruise and got turned down and referred to a specialist company called Biba! My husband who was diagnosed with MSA 3 years ago is still fairly independent, only needing a walking stick or rolator to walk. The operator mentioned she had to answer "No" to if the holiday was more than 2 months away. Should I only be getting insurance near to the departure date?

I use All Clear. Eye wateringly pricey and won’t give me an annual policy, but have never refused to insure me.
The MSA Trust has a leaflet on this I think.
if your husband has outstanding diagnostic assessments, it would be worth holding off on doing the insurance until they are concluded as pending investigations usually put the price up or Insurance is refused.
Otherwise I usually insure when I book so that I’m covered for cancellation. You have to declare any changes in your husband‘s condition to the insurance company anyway.

The MSA trust have a very useful factsheet which may be downloaded. This includes information about insurance as well as a host of other information. Jackie and I travelled extensively despite having to use a wheelchair for most of the time. Sadly the insurance company we used is no longer trading however we found the cost was okay provided we agreed to a reasonably large excess. We always thought we would just have to pay ourselves for any minor health issues abroad but wanted cover for anything more serious.

last year, as it was our first cruise, we used the insurance company that links with P&O. That was easy and there was no challenge to my wife’s conditions. You could try them if you are cruising with P&O. I can’t say if their insurance would be the lowest as we didn’t shop around as I was aware that I could have difficulty because my wife has multiple conditions in addition to MSA. Good luck and the cruise is fantastic.
