I have Polycythamy vera.Now I have high hemoglobin.7,28.Hematocrit is slightly increased-50.8PLT 420.But Leukozyten are increasing all the time.WBC was 16.46.Now-18.2

The doctor prescribed me Besremi 50 mg one week,second week-100mg

Is this a high dose?

Besremi is administered every other week. 100mcg is the standard starting dose, which is low. 50mcg is used when transitioning to Besremi from hydroxyurea. The maximum dose is 500mcg. The label instructions call for increasing dose 50mcg every other week until hematologic targets are met or the patients reaches the maximum tolerated dose. No all follow the rapid titration. Some prefer the low and slow approach. Some docs will increase to 200 or 250mcg and hold there to give the patient time to respond. Besremi takes time to do its work.

I chose to increase in 25mcg increments, which my care team was fine with. I reached max tolerable dose at 175mcg, which fortunately is effective for me.

It is recommended that you administer your first dose under medical supervision just in case of a very rare severe adverse reaction. If you have never administered a subcutaneous injection, you need to be trained anyway.
