Wondering if alternate days of Hydroxy would be a gentler (possibly )slow reduction in platelets?

Lower doses would result in fewer risks/adverse effects and lower/slower benefits. This can be achieved by an every-other-day regimen of 500mg - or by dosing using the brand name Droxia which comes in 200mg, 300mg, and 400mg. Assuming the brand name med is available where you are, the good news is that Droxia is not expensive. Generic HU only comes in 500mg.

Do be sure to review all of your options with your care team. HU is one of the two first-line treatments for ET/PV for people who need cytoreduction. It is up to you to decide whether a lower HU dose or another medication option is best for you.

I am in the States. You would have to check with the NHS un the UK to see if Droxia is in the formulary there.

I have been taking one 500 mg hydroxyurea every third day and that has led to a gradual decline in platelets without my having to feel tired from the hydroxyurea.
