Meningitis questionnaire for university project

Hi there everybody, I'm currently in my first year at university studying visual communication and for my next project it is a self negotiated brief - meaning you can choose what you want to do. Ive chosen to focus mine on meningitis experiences of my own and others. I would really appreciate if you have a spare few minutes to fill out this short form answering some questions about your experience to collect some data. It is completely anonymous and will help me in my research for my final pieces.

I'd be glad to fill out the info, but for the types of meningitis listed fungal was left out, which is a 'grave' mistake as that's precisely what I've dealt with. Viral/bacterial are much more common, but fungal seems overlooked and is why it is apparently hard to diagnose/treat effectively. Thanks
